A mother brought a 9-year-old girl to the emergency room at noon because the little girl accidentally fell on a bicycle 3 days ago and injured her perineum. At that time, a test, the right perineum was purple-blue, fortunately did not form a large subcutaneous hematoma, after simple outpatient treatment, I took the medicine home for observation.

Today to talk about vulvar hematoma, vulvar hematoma is mostly seen in children or young people, the reason is mostly to cross the railing, bicycle, walk the steel cable, slide along the handrail of the stairs, fall from the high and low bed, or directly from the vulva trauma, etc. Three years ago I had a Chinese New Year's Eve night to receive a young woman who was cleaning at home to clean up from a high place and ride a straddle injury caused by perineal hematoma, and the hematoma was removed in the hospital for the New Year's Day.

After the vulvar hematoma is severely impacted, the first thing you will feel is severe local pain and mobility. On examination, a lump is found to be raised in the injured area, which may be locally bluish-purple, tender and tender, and active bleeding occurs when there is a puncture in the skin of the vulva. If a huge hematoma is present, it can also compress the adjacent urethra, causing urinary retention. The size of the hematoma is related to the size of the ruptured blood vessel, the rupture of arteries and veins, and the length of time of injury.

Once a vulvar hematoma appears, it should be treated as soon as possible. If the vulvar hematoma is greater than 5 cm in diameter, or if it continues to progress and there is active bleeding, it should be surgically incised immediately, the blood clot should be removed, and the bleeding should be stopped. If the diameter of the vulvar hematoma is small and there is no continuous bleeding, local cold compresses, pressure bandaging, close observation can be given within 24 hours of injury, and the hematoma still does not continue to increase, and hot compresses are given after 24 hours to promote hematoma absorption. #我的门诊故事 #
