#Baoji Central Hospital# #Baoji Headlines # With the success of the first Da Vinci robotic surgery "debut" before the Lunar New Year, on the morning of February 9, the second case of Da Vinci robotic surgery carried out by our hospital: robot-assisted laparoscopic partial resection of the left kidney was successfully performed and showed its might!

The case is a 49-year-old woman who was hospitalized in the Urology Department of our hospital with a tumor in her left kidney. After examination, the tumor diameter is more than 4 cm, and the growth position is "tricky", most of the tumor is located inside the kidney, and it violates the renal pelvis and large blood vessels at the same time, which is extremely challenging for surgical operation. The traditional surgical method requires the removal of the left kidney, but the patient and his family are very willing to protect the kidney. In order to maximize the preservation of the patient's kidney, the urology organization discussed in a multidisciplinary consultation and decided to perform a partial resection of the left kidney. As we all know, the kidney itself is brittle, not only because of its abundant blood supply, the operation process is very easy to bleed, and the suture difficulty is high, the laparoscopic partial nephrectomy usually carried out nowadays is already a highly difficult kidney surgery, and the patient's preoperative renal tumor renal score is 10 points, which means that the use of conventional methods to do surgery is more difficult and risky. After combining the results of the patient's preoperative examination and full communication with the patient and his family, the department decided to use the da Vinci surgical robot to perform the operation on the patient.

The surgery proceeded as scheduled. Under the close cooperation of the "main knife" doctor, luo Xiaohui, deputy director of urology, doctor of medicine, and Wang Chenqing, the first assistant and attending physician, the robot's multiple mechanical arms rise and fall, revealing the kidneys, exposing the tumor, free of large blood vessels, isolating and cutting tumors, examining and repairing the renal pelvis and large blood vessel incisions in the wound... The whole operation is smooth and flowing, in one go. In just one and a half hours, the operation was successfully completed, and the bleeding was only 10 milliliters, not as much as a tube of blood sampled for blood tests.

"In the past, partial renal resection surgery was done by open surgery, which was more traumatic. Even if laparoscopic surgery is used, it will also be affected by tumor location and operating instruments, resulting in unsatisfactory tumor resection and suture, destruction of the structure around the kidney and other consequences that are not conducive to subsequent treatment, and in some cases, even the kidney will be turned into kidney surgery, deviating from the original intention of the operation. However, the da Vinci robot has a unique advantage over the traditional laparoscope, its robotic arm can be rotated in all directions of 360 degrees, can be flexibly operated in a narrow surgical space without dead angles, make up for the shortcomings that human operation can not be completed, and retain the original ecology of the kidney as much as possible. Luo Xiaohui said that the da Vinci surgical robot has a clear field of vision, can see the small blood vessels and fine residues of the tumor envelope that cannot be distinguished by the naked eye, maximize the retention of healthy tissue, remove the diseased tissue, the operation time is short, the trauma is small, and after the wound is completely healed, it is only a thin line that is difficult to distinguish with the naked eye, which minimizes the impact of postoperative scars on appearance.

It is understood that on February 11, the Department of Urology will also perform a da Vinci surgical robot-assisted laparoscopic radical prostate cancer resection for a prostate cancer patient. In the following days, three patients will be operated on in turn. In the next step, our hospital will also expand the field of surgery, so that da Vinci surgical robots can show their might in the treatment fields of thoracic surgery, gynecology, anorectal surgery, oncology surgery, general surgery, hepatobiliary, pancreatic and spleen surgery, etc., and provide top-notch, high-quality and efficient surgical services for more patients.
