The clock is set back to December 26, 2021, Master Wang, who was working at the construction site, accidentally fell high and was penetrated by a steel bar about 4 centimeters in diameter and about 60 centimeters in length through the perineum on the ground... After being sent to the emergency department of Shanghai Tenth People's Hospital, the team of the Trauma Emergency Center, together with the Department of Orthopedics, Urology, Abdominal Pain Center, ICU, Interventional Department and other departments, carried out MDT (multidisciplinary team diagnosis and treatment) treatment, with the assistance of the fire department, the steel bar outside the body was first truncated, and then the steel bar was carefully removed under the guidance of three-dimensional image modeling, and no secondary damage was caused to important blood vessels, nerves and tissue organs.
After three hours of meticulous surgery, the patient was finally turned around, and on the seventh day after the operation, the patient was steadily transferred to the orthopedic general ward and all drainage tubes were removed, and he is currently recovering well and is expected to be discharged from the hospital soon.
The scaffolding accidentally fell high, and the dirty steel bar plunged into the body
At two o'clock in the afternoon, Master Wang, 41, was busy on the construction site as usual, standing on a scaffolding two meters above the ground, nailing wooden planks, not fastening safety ropes, and wearing only hard hats, but such negligence caused serious consequences.
Due to accidental stepping into the air, he fell on his back from the scaffolding, two meters is not too high, but the terrible thing is that there is a steel bar on the ground, which plunges into his body. The rebar entrance is located at the base of the right thigh, penetrating the body and penetrating from the right perineum, the scrotum is badly damaged, even the testicles are dislocated... Not only was his lower body severely damaged, but Master Wang's limbs and head and face were also damaged. The workers immediately sent him to the hospital for treatment, but the local primary hospital did not have the conditions for treatment, and finally the ambulance sent him to the Trauma Emergency Center of the Tenth City Hospital.
Before the patient arrived at the hospital, the emergency medical team of the Tenth Hospital had contacted the medical institution and family members of the first diagnosis to learn more about the condition and prepare for rescue - the trauma center and the director team of Zhang Chunlin of the Department of Orthopedics, the director team of Yanyang of the Department of Urology, as well as the abdominal pain center and the interventional department jointly held consultations.
Firefighters rushed to the rescue, and the white soldiers underwent fine surgery
After Master Wang arrived at the Trauma Center of the Tenth Hospital, the medical team found that the situation was more shocking than expected - in order to avoid secondary injuries, the workers did not dare to move the steel bar, and the two ends of the steel bar were protruding from the body by more than ten centimeters, and a large amount of cement debris was attached to it.
Ct and other related examinations were perfected as soon as they were admitted to the hospital, and the diagnosis was clearly defined as body steel penetrating injury, open perineal injury, scrotum injury, and combined with limb injury and head contusion. In order to increase the safety factor when the rebar is removed later, Professor Zhang Chunlin's team decided to first ask the fire department to assist in truncated the rebar protruding outside the body. After the firefighters arrived at the scene, they used a chainsaw to cut off about 10 centimeters of steel bars at each end of the body, providing space for doctors to carry out treatment.
The Trauma Center and the Department of Urology immediately perform emergency inguinal perineal exploration + perineal penetrating foreign body removal + thigh sciatic nerve exploration + debridement and suturing.
Extensive contusions can be seen during surgery, the length of the through-wound is about 25 cm, and the creepy thing is that the steel bar is even less than 1 cm closest to the left femoral artery, and if it deviates by a fraction, the major bleeding caused by the femoral artery injury will be immediately life-threatening.
At the same time, one side of the reinforcement is close to the sciatic nerve, and the other side is close to important tissue organs such as vas deferens and penile cavernosum, and once the operation is not careful, it will face irreparable disability and dysfunction.
Zhang Chunlin, director of the Department of Orthopedics, and Yan Yang, director of the Department of Urology, were bold and careful, leading Dr. Hu Jianping to carefully separate the steel bar and local tissues, and finally completed the operation safely after 3 hours. After surgery, the length of the steel bar pierced into the body is about 60 cm and the diameter is about 4 cm.
After the operation, Master Wang was sent to the ICU, the anesthesia process was completed, his consciousness was fully restored, he was able to talk to the medical staff clearly, and even remembered the details of the injury process, but this did not mean that the danger had passed. Due to the large amount of dirt attached to the steel bar that pierced into the body, coupled with the stress caused by the trauma, Master Wang's body temperature increased after the operation. On the one hand, the medical team closely monitored, on the other hand, accurate preventive medication, and finally helped him successfully pass the postoperative infection barrier.
On the seventh day after the operation, Master Wang's vital signs were stable, his body temperature and so on tended to be normal, and he was smoothly transferred from the ICU to the orthopedic general ward and all drainage tubes were removed. Director Zhang Chunlin said that the injured recovered well and is expected to be discharged from the hospital in about a week, and can be healthy with their families to welcome the New Year.
