Low concentration

atropine

Atropine is actually very common, especially when mydriasis is refracted, and what we call "slow dispersion" is the use of atropine. Refraction of ciliary muscle paralysis with 1% atropine is also an indispensable step in the refraction of children under 7 years of age, such as myopia, esotropia, and high hyperopia.

Studies in recent years have shown that different concentrations of atropine eye drops have a significant effect on delaying the development of myopia, although the effect of low concentrations of atropine is not as good as high concentrations, but the degree and effect of adverse reactions that children can tolerate, most of them now use low concentrations of atropine.

Latest research

00

In August 2021, the results of a study published in Ophthalmology, a well-known journal of optometry, compared the effects of 0.05% atropine, 0.01%, and 0.025% atropine in controlling the progression of myopia.

The results show:

Poor response to treatment with low concentrations of atropine in 0.025%, 0.01%, and 0.05% is associated with younger age.

After observing greater efficacy without significant adverse effects on quality of life (QOL), 0.05% atropine was used as the optimal concentration for long-term myopia control, with an effect of more than 0.01% and 0.025% atropine.

Adverse reactions

01

Dilated pupils, photophobia, and blurring near and far.

The effect of atropine on the eye is mainly to relax the ciliary muscle, but it will incidentally cause dilated pupils, and under the same conditions more light enters the eyeball, so it will be photophobia. The change in pupil size in low concentrations of atropine is very small relative to that of high concentrations of atropine, resulting in a faster recovery of transient myopia blur.

May cause allergy problems.

A small proportion of children and adolescents develop problems such as allergic conjunctivitis and allergic blepharitis. Once the allergic problem occurs, it can disappear after active treatment after discontinuation of the drug.

Intraocular pressure changes.

After atropine use, the pupils are dilated, which may cause an increase in intraocular pressure. However, foreign studies have shown that there is no significant difference in intraocular pressure after 3 years between the two groups of children who use and do not use atropine.

Precautions

02

If the child is in a period of rapid myopia development and wants to try to use low-concentration atropine, you must go to a regular hospital to first confirm whether it is suitable for using low-concentration atropine. It is also important to note that it is extremely important to check the visual function of binocular eyes (eye position, collection, dispersion, adjustment, etc.) in children before using low-concentration atropine.

After the eye is spotted, press the inner corner of the eye for 3 minutes to prevent atropine from entering the nasal cavity from the tear point and being absorbed.

Although it is ordered at night, some children will still have different degrees of pupil dilation and outdoor photophobia on the second day, and they can wear a sunhat or a pair of glasses that can automatically change color for the child. Some children have a blurred appearance of looking closely due to the relaxation of the ciliary muscles and the decline in the ability to adjust the eyes after use. Therefore, it is necessary to go to a medical institution with clinical research qualifications and guide the use under the close supervision of the doctor.

when

Start using atropine

03

It's not that myopia is about to drip

It can also be dripped when it is not myopic

Now the domestic prevention and control of children's myopia has been very good, we have been screening the child's vision from the age of three, will establish a set of relatively complete development files for them, that is to say, we will give the child to measure the degree of his eyes every year. If the child's farsighted reserves are relatively small, and his annual reserves decrease very quickly, even if he does not have myopia, he can also order low concentrations of atropine.

Then the child who has been nearsighted has not grown his eyesight for several years, and I can also not order atropine. If we observe that his myopia grows rapidly, for example, by more than 100 degrees in a year and the eye axis grows by more than 0.3 mm a year, then the intervention of atropine is necessary at this time.
