A shift of personnel configured according to the 8-hour work often has to work 16 hours, or even 24 hours, and overtime is very frequent," she said in an interview with the media, "but it is not allowed to pay any overtime pay and labor costs." ”

As a result, even if governments at all levels attach great importance to the construction of the CDC system and increase the staffing of the CDC, the personnel "cannot be recruited and retained". This phenomenon not only occurs in grass-roots CDCs such as prefectural and municipal levels and county levels, but also exists in the National CDC.

What is the reason for this embarrassing situation? Wu Fan said bluntly that the main reasons are high requirements for entering the door, low salary level, low salary level, heavy work tasks, and poor development prospects.

In the survey, she found that due to the nature of the profession, the CDC has higher requirements for the professional background and comprehensive quality of the practitioners, and the state requires that the business personnel of the CDC should reach more than 80%, so the recruits are basically bachelor's degrees in preventive medicine and related majors.

In national and provincial disease control institutions, and even in some developed areas, the entry threshold will be higher, mainly with postgraduate degrees.

On the other hand, the per capita performance wage level of cd-off centers in various places is generally lower than the salary level of local civil servants, and the per capita salary of medical institutions at the same level is even more disparity. This is because the disease control agency, as a public welfare first-class public institution, is fully guaranteed by the finance and implements performance wages. However, the total amount of performance pay that should be used as the government's "bottom" for disease control agencies is controlled as a revenue "cap line" in the implementation of the government, which is contrary to the original intention of setting up the performance wage system.

In addition, monitoring and early warning, risk assessment, rapid emergency response, evidence-based decision-making, etc. all require disease control agencies to carry out a large number of applied basic research to form a talent reserve and technical reserve. Wu Fan said that from the national to the local level, scientific research services are required to be included in the total performance wage accounting of disease control institutions, which is contrary to the current general trend of the state to stimulate scientific research and innovation.

Even participating in continuing education training, publishing articles, etc., is regarded as a need for personal title promotion, and the cost requires individuals to bear it in full, which is very unfavorable to the rapid development of the cause of disease control.

Call for a thorough reform of the current remuneration system for disease control agencies

Wu Fan submitted a proposal this time, calling for a thorough reform of the current salary system of disease control agencies and an increase in the salary level of disease control personnel.

She recommends:

First, the departments of human resources and social security, finance and other departments refer to the per capita income level of comprehensive medical institutions at the same level in various places, approve the average performance wage level of disease control institutions, and form a long-term mechanism for synchronous growth.

Second, improve the subsidy system for emergency handling of public health emergencies, and calculate and issue overtime pay to personnel undertaking emergency response tasks on an annual basis, and the funds shall be borne by the finance at the same level.

Third, it is clear that scientific research services are not included in the total performance wage, and disease control personnel are allowed to receive reasonable and compliant scientific research labor remuneration other than performance wages.

She believes that only by starting from the aspects of post setting, salary system, incentive mechanism, etc., and strengthening the construction of the disease control talent team, can the disease control team be improved in terms of number and quality.

In Wu Fan's view, the cultivation of talent team is the key to the construction of the public health system. From June to September 2019, before the outbreak of the new crown epidemic, she led the convening of 12 members of the social welfare and social security sectors of the National Committee of the Chinese People's Political Consultative Conference to go to the whole country to conduct a special investigation on "implementing prevention first and effectively strengthening the construction of public health system".

After the survey, Wu Fan formed a 120-page research report summarizing a series of institutional and institutional problems related to the mainland's public health system. At the two sessions of the National People's Congress in 2020, combined with the outbreak of the new crown epidemic, Wu Fan improved and condensed this research report to 28 pages, submitted it to the general assembly and relevant government functional departments, and the conference also conducted special consultations on this sector.

This year, she once again stressed the need to establish a public health physician system and build a fundamental system for the cultivation of disease control talents. She said that most of the talents in the disease control industry graduated from the "public health physician" major, at present, the training of such talents should be based on the standardized training pilot, according to the characteristics of the local optimization of the training model, curriculum system and evaluation standards, to ensure that students are trained into "public health, clinical, emergency" compound talents.

She also suggested reforming the existing title evaluation system of disease control agencies to increase the weight of public health practice achievements in personnel promotion; it is recommended to broaden the professional span of posts and include more information science, big data and artificial intelligence, mathematical statistics, social sciences and other professional fields into disease control positions.

Solve the "pain points", "breakpoints" and "weaknesses" step by step. Wu Fan said that to adhere to the "bottom line" of disease prevention and control, the most important thing is to increase the construction of this team of disease control, so that this invisible team can always be at the forefront, "open the way" for the health of the people, and support the healthy development of the entire society.
