Recently, the Beijing Winter Olympics opened in the Olympic motto of "higher, faster, stronger and more united", and the bright posture of the ice and snow athletes became the most eye-catching scenery. At the same time, there is a saying that has also regained the public's attention - "life is in sports, athletes have a slow heart rate, so the slower the heartbeat, the healthier and longer." This claim actually lacks scientific basis.

Even for athletes with long-term training and a strong heart, the slower the heart rate, the better. Too low a heartbeat rate may lead to ischemia and hypoxia throughout the body, causing chest tightness and shortness of breath, dizziness, memory loss and other symptoms, and heartbeat stopping for more than 5 seconds may also cause syncope, which in the long run will lead to enlarged heart, decreased heart function and even sudden death.

Pacemakers are the only way to treat bradycardia, and leadless pacemakers are the latest advance in this area. Compared with traditional pacemakers, the leadless pacemaker is like a small "capsule", without wires and subcutaneous pockets, directly installed in the heart, aesthetically pleasing and more comfortable.

At the HRS conference in Boston, USA, in July 2021, research on the clinical application of Micra AV in the real world has yielded results. Micra AV is the world's first dual-chamber leadless pacemaker that enables accelerometer-based atrioventricular synchronized pacing, i.e. synchronous pacing of the ventricles after sensing atrial mechanical contraction signals. The researchers collected postimplantation follow-up data from 2413 patients and found that the three major parameters of Micra AV pacing performed well and steadily, with an estimated average service life of > 10 years, and about 3/4 of the simultaneous pacing was provided for pacing-dependent patients, which greatly benefited. It can be seen that Micra AV is another important milestone in the development of pacing therapy, and it is expected that more patients can be benefited in the future.

Patients with atrial fibrillation reduce the risk of ischemic stroke after abstaining from alcohol

Ischemic stroke is an important complication of atrial fibrillation and a key factor threatening the safety of patients with atrial fibrillation. In the process of preventing ischemic stroke in patients with atrial fibrillation, lifestyle interventions have received more and more attention, which determines the future quality of life and clinical prognosis of patients to a certain extent.

Recently, Professor Eue-Keun Choi of Seoul National University Hospital in South Korea published a large retrospective clinical study based on the Korean National Medical Statement and Health Examination Database in the European Heart Journal to assess the relationship between drinking status after newly diagnosed atrial fibrillation and the risk of ischemic stroke (Eur Heart J. 2021 Dec 7; 42(46):4759-4768)。 The study included a total of 97 869 patients newly diagnosed with atrial fibrillation between 2010 and 2016, and divided patients into three groups based on their drinking status before and after the diagnosis of atrial fibrillation: the non-drinking group, the abstinence group after the diagnosis of atrial fibrillation, and the continuous drinking group. At 5 years of follow-up, the risk of stroke was significantly reduced in the abstinence and non-drinking groups compared with the continuous drinking group (the difference in incidence after IPTW was -2.03 [-3.25, -0.82] and -2.98 [-3.81, -2.15], respectively, compared with the continuous drinking group).

The study suggested that continued alcohol consumption was strongly associated with an increased risk of ischemic stroke in patients with newly diagnosed atrial fibrillation, and that abstaining from alcohol after atrial fibrillation was confirmed reduced the risk of ischemic stroke. Alcohol consumption control should be actively advocated as part of a comprehensive treatment regimen for atrial fibrillation to improve patient outcomes.
