A few days ago, the Shandong Provincial Health Commission and the Provincial Department of Education published the "Notice on Announcing the Affiliated Hospitals of Undergraduate Colleges and Universities" according to the application, which re-identified the affiliated hospitals of Shandong universities. At the same time, it is clear that in addition to the 96 hospitals listed in the list, the affiliated hospital relations established by Shandong universities and relevant hospitals will be cancelled.

The document requires that from the date of the issuance of the document, all colleges and universities should immediately clean up the hospitals that have not been identified as affiliated hospitals in accordance with the requirements of this notice, give delisting or cancellation agreements, and the relevant hospitals shall no longer use the name of "**Affiliated Hospitals of Universities", complete rectification within 10 days, and report the clean-up situation to the Provincial Department of Education and the Provincial Health Commission respectively.

▎The university is listed as an affiliated hospital, and chaos is emerging

In recent years, it has become common for mainland universities to blindly list affiliated hospitals, which has triggered a series of chaos.

"A university has 23 affiliated hospitals", "universities list 9 affiliated hospitals on the same day", "a hospital 'one daughter marries more' to become an affiliated hospital of many universities", it is not uncommon.

Once upon a time, the affiliated hospitals of medical colleges and universities were "rare" in medical institutions at home and abroad, and they undertook the important task of clinical teaching in schools, on the one hand, to train medical students, on the other hand, to produce scientific research results, and belonged to the "academic school" of the bellwethers.

However, after years of "staking-off listings", there are more and more "xx university affiliated hospitals" that are "indiscriminate but not refined" and even have no real name.

"Many colleges and universities across the country have established affiliated hospitals across cities and even provinces. However, there is no affiliation between the non-directly affiliated hospitals and the universities in terms of personnel, management, finance, etc., but only hang up a name, and the two are 'famous but not real'. Universities are scrambling to occupy medical resources, and the practice of staking land has far deviated from the fundamental purpose of the affiliated hospitals of universities to complete clinical teaching tasks and provide services for clinical teaching in colleges and universities. From the perspective of affiliation, the affiliated hospitals in the mainland are roughly divided into two types: directly affiliated hospitals and non-directly affiliated hospitals. The former is affiliated with medical colleges and universities, and the main decision-making power is in the "parent" universities; the latter is only a cooperative relationship with the universities, and the higher authorities are still the local health commissions.

However, under the blind pursuit of quantitative "flexibility", the relationship between universities and hospitals is becoming more and more ambiguous and ambiguous.

Taking a medical school in a certain place in the Yangtze River Delta as an example, the first affiliated hospital is a directly subordinate hospital, and the actual supervisory unit of the second affiliated hospital is a central enterprise; while in another medical school, the affiliated hospitals include both directly subordinate hospitals, foreign hospitals cooperated by foreign-funded companies, and social hospitals invested by private capital.

▎ Arts, science, industry, and peasants are on the battlefield and run medicine!

The reason why colleges and universities are so eager for "affiliated hospitals" is because there are huge "hidden benefits" behind them.

Taking the National Natural Science Foundation of China, which has attracted the attention of domestic researchers, as an example, from 2000 to 2019, the total amount of funding from the medical part of the National Natural Science Foundation of China increased from 112 million yuan to 4.651 billion yuan, an increase of about 40 times.

This also means that the stronger the medical strength of the university, the more able to occupy a greater advantage in the number of projects and the amount of funding of the National Natural Science Foundation.

In 2019, Shanghai Jiao Tong University ranked first in the total number of approved projects, and the number of medical projects accounted for 52.71%. Fudan University, Sun Yat-sen University, Huazhong University of Science and Technology, Central South University, Peking University, Sichuan University and other universities accounted for about 50% of the number of medical projects in that year.

Among them, the medical schools of various universities and their affiliated hospitals are undoubtedly the "big households" in the declaration of fund projects.

In this context, the larger the scale of the medical school and the greater the number of affiliated hospitals, the more scientific research projects, funding amounts, and academic papers that can be included in the name of the university, and the more they can further promote the school's rise in the list of various university rankings.

As a result, the arts, sciences, engineering, and peasants joined forces, and mainland colleges and universities set off a boom of "running medicine."

According to incomplete statistics, as of July 2020, of the 42 first-class universities, 35 have made moves in the construction of medical disciplines.

▎Rectify the blind listing of "affiliated hospitals" in universities

When will the hospital be listed on the university chaos?

"In today's coordinated development of colleges and universities and hospitals, all kinds of chaos must be controlled and resolved in a timely manner in order to ensure the continuous advancement of the mainland's medical cause."

At the beginning of 2021, Ge Junbo, member of the National Committee of the Chinese People's Political Consultative Conference and academician of the Chinese Academy of Sciences, submitted a motion to "stop" the random listing of affiliated hospitals in colleges and universities, and put forward three specific suggestions:

First, the responsibilities of listed hospitals need to be clear, and the qualifications of listed hospitals should be strictly controlled; second, the number of listed hospitals should be controlled and the idea of quantity theory should be resolutely eliminated; third, after the hospital is listed, quality control and strict management can be a win-win situation for both sides.

The Ministry of Education also clearly pointed out in the "Reply to Recommendation No. 3381 of the Third Session of the Thirteenth National People's Congress" that affiliated hospitals are an important basis for clinical medical education, an important carrier for the construction of clinical disciplines, and an indispensable and important part of medical colleges, but it is not possible to blindly increase the number of affiliated hospitals.

In 2021, the General Office of the Ministry of Education, the General Office of the National Health Commission, and the Office of the State Administration of Traditional Chinese Medicine jointly issued the Notice on Carrying Out Special Governance and Rectification work in hospitals affiliated to universities.

At the same time, the Education Department and health commissions of Shandong, Henan, Shaanxi and other provinces across the country have sent expert groups to carry out special governance and rectification work related to university-affiliated hospitals.

Now, the results of Shandong Province's treatment have come out first, which means that the boots have officially fallen and the alarm bells have begun to sound.

After so many years of "staking corners" and burying our heads in a rush, it is time for our universities to stop and ask themselves: What is the purpose of so many "affiliated hospitals"?
