According to data from the Chinese Society of Gerontology and Geriatrics, the number of elderly people over the age of 60 in 2020 is 255 million, and the elderly over the age of 60 will account for 35% of China's total population in 2050. At present, there are 3.8 million new cancer cases and about 2.29 million deaths per year.

The quality of life at the end of life is the most easily overlooked happiness problem, but many people do not know that whether it is a seriously ill patient or the elderly in the last journey of life, they are facing great psychological pressure. Among them, in addition to the inescapable fear of death, in fact, more from other aspects: such as regret for unfulfilled wishes, such as the psychological burden of thinking that they are a burden to the family, and so on.
Therefore, the whole society urgently needs to provide physical, psychological, spiritual and other care and humanistic care services for terminally ill or elderly patients before dying, so as to help patients die comfortably, peacefully and with dignity.
According to statistics, Tongzhou District plans to set up 19 community health service centers, 137 service stations, 18 community health service centers and 66 service stations (including 53 government offices and 13 social offices), 2639 on-the-job health technicians in community health service centers (stations), including 953 physicians (including 524 general practitioners), 818 registered nurses, 4.959 million total diagnosis and treatment of practicing (assistant) community health service institutions, and 1228 outpatient services.
However, up to now, no public primary medical institution in Tongzhou has set up a hospice ward. The issue of palliative care needs to be addressed urgently.

The cornerstone of hospice care is in public grass-roots hospitals, the advantages of public grass-roots hospitals are supplied in wards, public second-level and third-level hospitals, a bed is difficult to find, even if the pilot opens a hospice ward, it can only take out a very limited number of beds to put into use. In stark contrast, there are 22 public first-level medical institutions in Tongzhou District public primary hospitals, with a total of 605 beds, 420 actual beds, and a ward utilization rate of 8.74%. That is, there are 91.26% of idle beds.
At the same time, grassroots hospitals are deeply rooted in the community to facilitate family visits, especially suitable for the pilot hospice ward. Therefore, according to the use of wards in Tongzhou public grass-roots hospitals, it is recommended to take out idle wards to open hospice wards.
At the same time, Cao Jian suggested that the pilot project of hospice care needs to increase investment and achieve both talent training and introduction.
Carrying out hospice care pilots requires relevant departments at all levels to attach great importance to and support, increase investment, increase efforts, strengthen policy and financial support, and take multiple measures to support public grass-roots hospitals to carry out hospice ward pilot work.
In addition, it is necessary to build a high-quality hospice team. In view of the current situation of grass-roots hospice personnel, Cao Jian suggested that we should adhere to the introduction and training at the same time. It is necessary to intensify the training of grass-roots hospice personnel, introduce experienced and outstanding talents, and rapidly increase the effective force. At the same time, it is necessary to explore the establishment of a set of training mechanisms as soon as possible, implement systematic training for existing personnel, and carry out long-term and in-depth cooperation with relevant institutions to achieve "self-hematopoiesis".
