Huaibei "2021525" poisoning and suffocation accident was closed, resulting in 4 deaths

On December 23, the Emergency Response Department of Anhui Province announced the "Provincial Government Approval and Closure of the Case in Xiangshan District, Huaibei City "2021•5•25" Large Poisoning and Suffocation Accident": Recently, the People's Government of Anhui Province approved the "Investigation Report on the Larger Poisoning and Suffocation Accident in Xiangshan District of Huaibei City "2021·5·25", and agreed with the investigation team's determination of the cause and nature of the accident, the treatment suggestions for the responsibility identification and responsible units, and the preventive measures and rectification suggestions proposed.

At about 9:00 a.m. on May 25, 2021, a poisoning and suffocation accident occurred at the construction site of Renmin West Road in Xiangshan District, Huaibei City, resulting in 4 deaths and direct economic losses of 3.139 million yuan.

The investigation report determined that the accident was a large production safety liability accident. The direct cause of the accident was: the construction personnel violated the relevant provisions of the "Interim Regulations on safety management and supervision of limited space operations in Anhui Province", entered the accident well without authorization without performing the approval procedures and without ventilation, testing and personal protection, due to the high concentration of toxic gases such as hydrogen sulfide in the well, resulting in the construction personnel drowning in the sewage after poisoning when they went down the well to take tools, and other personnel blindly saved people without safety protection, resulting in the expansion of accident casualties. The indirect causes of the accident are: first, the relevant participating units have not implemented the main responsibility for safe production, and the construction acceptance is not standardized; second, the local government and its relevant departments have not supervised the project in place.

The investigation report pointed out that the 2 relevant responsible persons suggested that the public security organs file a case for investigation; the 9 responsible persons and 5 responsible units related to the accident were given corresponding administrative penalties; and after filing and investigating the case, the discipline inspection and supervision organs have held accountable the 12 party-member leading cadres and 4 units transferred by the investigation team.

In the recommendations on accident prevention measures, the investigation report requires that Huaibei City and Xiangshan District should improve their political positions and strengthen the implementation of safety responsibilities. Truly put safe production in a prominent position, conscientiously perform safety supervision duties, tighten the chain of responsibility at all levels, strengthen source prevention and control, and systematically improve the intrinsic safety level of the industry in this field. Second, we must strictly abide by the bottom line of the law and implement the main responsibility of enterprise security. The relevant participating units should implement the main responsibility of the enterprise, supervise the construction and supervision units to strengthen safety management; the construction units should strengthen the safety production management of the project site to ensure the safety of construction; the supervision units should conscientiously perform their supervision duties and implement the relevant safety management systems. Third, we must profoundly draw lessons and comprehensively investigate and manage all kinds of hidden dangers. It is necessary to immediately carry out special rectification of safety in the field of construction and construction, comprehensively carry out risk identification in limited space work sites, supervise and urge construction enterprises to carry out investigation and rectification of potential safety hazards, and take effective measures to urge enterprises to rectify and put in place in a timely manner. Fourth, it is necessary to strengthen publicity and education and enhance safety awareness and ability. It is necessary to increase the intensity of publicity and training in limited space, urge enterprises to seriously carry out safety education, implement pre-job training, and carry out special training, so that practitioners can master the safety and protection knowledge of limited space operations.
