Is enteric-coated aspirin eaten on an empty stomach or after a meal? For so many years, you may have been eating the wrong thing!

1

Eating on an empty stomach can maximize the efficacy and safety

Recently, Liu Meilin, Wang Xiting, etc. of peking university first hospital used magnetron capsule gastroscopy to track the drug, and found that after taking intestinal solvent-based aspirin on an empty stomach, the drug did not disintegrate in the stomach, and only began to disintegrate after entering the small intestine.

The authors believe that if the patient takes enteric-coated aspirin after a meal, on the one hand, food or gastric residue increases the pH level in the stomach, which may cause enteric-coated aspirin to collapse in the stomach in advance, and gastrointestinal peristalsis will also be enhanced after eating, and the enteric-coated coating is more easily damaged under mechanical squeezing, further increasing the risk of gastric mucosal damage.

On the other hand, eating significantly prolongs the time of gastric emptying, which may cause enteric-coated aspirin to remain in the stomach, unable to quickly enter the small intestine for decomposition and absorption, delay the timing of anti-platelet aggregation, and reduce the efficacy of antiplatelets.

In view of this, the authors note that patients should be advised to take intestinal solvent-based aspirin on an empty stomach to maximize the efficacy and safety of antiplatelets.

Taking aspirin at the right time can reduce its side effects

Aspirin can treat a variety of diseases, many people with cardiovascular and cerebrovascular diseases will take it for a long time, but its side effects can not be easily ignored.

The most common side effect of oral aspirin is damage to the gastrointestinal tract, causing epigastric discomfort, indigestion, stomach pain, nausea, vomiting, etc., and some people may have constipation after taking aspirin. Taking aspirin in large doses can cause erosive gastritis, ulceration, and bleeding.

Many patients with cardiovascular and cerebrovascular diseases need to take aspirin for a long time, but they are also worried about the side effects of the drug and their dependence on the drug, so many patients will consider discontinuing the drug.

In fact, although aspirin has certain side effects, the risk of these side effects is relatively low. Studies have shown that discontinuation poses a greater risk than taking aspirin. Therefore, for patients with cardiovascular and cerebrovascular diseases who need to take aspirin for a long time, it is not recommended to stop the drug casually. In addition, as long as the right time to eat, you can reduce the damage of aspirin to the stomach.

03

Aspirin should be personalized

Aspirin enteric-coated tablets have an enteric-coated membrane on the surface. With this enteric-coated membrane, the tablets can resist stomach acid, and decompose in the alkaline environment of the duodenum, so that the harm of side effects is reduced. But different people should also personalize the medication.

Previous studies by Peking University First Hospital have shown that for the elderly who are ≥ 60 years old, taking aspirin can significantly inhibit the platelet aggregation rate, and the short-term safety is better, of which the adverse reactions of enteric-coated extended-release dosage forms are lower. The researchers recommend that the dose of aspirin used in elderly patients should be individualized, and that individuals with a high risk of bleeding should be cautious about primary prevention with aspirin, and if necessary, they should be used as prescribed by a doctor.
