"There are problems such as insufficient number of rural medical institutions in the mainland, obvious aging of personnel, and low level of diagnosis and treatment, and we should effectively solve the treatment of rural doctors, and strengthen many measures such as personnel training and performance appraisal to comprehensively improve the health services of rural medical institutions." Professor Huo Yong, a member of the National Committee of the Chinese People's Political Consultative Conference and the First Hospital of Peking University, told the People's Daily health client reporter.

At this year's two sessions, Professor Huo Yong, member of the National Committee of the Chinese People's Political Consultative Conference and the First Hospital of Peking University, proposed a proposal entitled "Further Improving Health Services in Rural Medical Institutions and Improving the Sense of Access to Medical Treatment for the Grassroots Masses"; in addition, the "Proposal on Strengthening Rural Public Health and Medical Capacity Building in Mainland China" submitted by Professor Yang Jiefu of the National Committee of the Chinese People's Political Consultative Conference and Beijing Hospital; the "Proposal on Increasing Grassroots Medical and Health Support and Avoiding the "Siphon Phenomenon" of Medical Resources" submitted by Huang Yuguang, member of the National Committee of the Chinese People's Political Consultative Conference and director of the Department of Anesthesiology of Peking Union Medical College Hospital. The three CPPCC members all put forward their own views and suggestions on improving primary medical and health services.

Huo Yong: Improve health services in rural medical institutions

With the change of national lifestyle, the impact of some disease risk factors on the health of the people has become more and more significant, and cardiovascular disease accounts for the first cause of disease death, and the "China Cardiovascular Health and Disease Report 2020" shows that the prevalence of cardiovascular disease in China is in a continuously rising stage. It is estimated that the number of patients with cardiovascular diseases is 330 million, and the mortality rate of rural cardiovascular diseases has exceeded and continued to be higher than the urban level since 2009.

At this stage, there are many shortcomings in the service of rural medical institutions, and the level of diagnosis and treatment of rural doctors is insufficient; rural medical institutions on the mainland lack the necessary medical equipment and basic drugs; the number of rural doctors is seriously short, the treatment cannot be stable, most rural doctors lack systematic training, and the level of diagnosis and treatment is low.

Grassroots medical institutions, especially township health centers and village clinics, are the medical resources at the "doorstep" of the people. Huo Yong suggested that local health administrative departments increase policy support; improve the basic specialized capacity of rural medical institutions, promote the creation of chest pain treatment units in grass-roots medical institutions such as township health centers in the medical community; improve the welfare treatment of rural doctors, improve the configuration of facilities in rural medical institutions, rescue equipment such as electrocardiogram machines, defibrillators, and ensure the basic use of major chronic diseases such as cardiovascular and cerebrovascular diseases; actively carry out training, improve the level of rural medical diagnosis and treatment, and implement performance appraisal.

Yang Jiefu: Strengthen rural public health and medical capacity building in the mainland

With the acceleration of the urbanization process on the mainland, the large-scale flow of rural population to the city has led to the "hollowing out" of the rural population, and the medical treatment of empty nest elderly and left-behind children has become the main problem facing rural medical care. At the same time, many elderly people living in rural areas are not accustomed to seeing a doctor in the hospital, and when they are delayed until the end of the disease, they are limited by the ability of rural medical services, making it difficult to get immediate and proper diagnosis and treatment of the disease. Therefore, Yang Jiefu mentioned that it is urgent to strengthen the capacity building of public health and medical care in rural areas on the mainland.

Yang Jiefu's proposal proposes to strengthen the capacity building of rural public health and medical care in the mainland from five aspects. First, adjust the structure of fiscal expenditure from the national policy level and increase investment in rural public health and medical care; second, strengthen the support of hospitals above the second level in cities to support rural areas; third, actively guide the flow of medical talents to rural areas; fourth, strengthen the construction of rural public health talents; fifth, strengthen the construction of rural informatization and promote hierarchical diagnosis and treatment.

Promoting the healthy development of primary medical and health care is still an important measure to achieve a healthy China. The shortage of staffing in grass-roots medical institutions is serious, and the competence of medical services is generally low. To a certain extent, there are problems in the construction of the team of doctors in county medical institutions (including county and rural areas), such as shortage of personnel, lack of talent incentive mechanism, and low personnel salaries. The training of professionals in grass-roots medical institutions is not standardized, and grass-roots hospitals cannot be retained. ”

Huang Yuguang's proposal suggests that public hospitals cannot take the road of enterprise expansion, and governments at all levels cannot manage public hospitals as simple enterprises. The "baton" and support of the policy have been transferred to grass-roots medical institutions; the information system has been fully utilized to carry out telemedicine, the high-quality medical resources of large hospitals have been shared with grass-roots hospitals, and multiple measures have been taken to promote the construction of a hierarchical diagnosis and treatment system; the establishment of county hospitals as the leader and a close consortium of county medical institutions has been established, and it is recommended that the establishment and management of township health personnel be incorporated into the management system of county hospitals; and the establishment and improvement of the talent incentive mechanism for professionals in grass-roots medical institutions has been established and improved to improve their remuneration and treatment.
