What is a Family Doctor Contract Service?
The family doctor contract service is based on the general practitioner as the core, supported by the family doctor service team, through the way of signing the contract, to promote the general practitioner (clinical) doctor with the family doctor conditions and the contracted family to establish a long-term and stable service relationship, in order to maintain the health of the contracted family in the whole process, to provide safe, convenient, effective, continuous and economical basic medical services and basic public health services for the contracted family and individual. Who is the family doctor?
At present, family doctors in mainland China are mainly composed of registered general practitioners (including assistant general practitioners and general practitioners of traditional Chinese medicine) in township health centers, as well as competent clinicians and rural doctors. In principle, services are provided in the form of a contracted team, including family doctors, nurses, public health doctors, rural doctors, etc. Physicians in hospitals above the second level provide technical support and business guidance.

What does a family doctor do?
Family doctors are mainly responsible for preventive health care, diagnosis and treatment and referral of common diseases, patient rehabilitation and chronic disease management, health management and other integrated services. Family doctors take people as the center, take the family as the unit, and take the maintenance and promotion of overall health as the direction, provide long-term contract care, and integrate individual and group health care.

What services do you enjoy after signing a contract?
As long as you sign up for the basic package, you can enjoy at least the following ten basic services:
1. Establish resident health records. Family doctors establish resident health files for them, collecting residents' past history, family history, as well as basic health information such as health examinations, lifestyle, health status, and disease medication. Help them keep, maintain, and update their health records.
2. Priority appointment appointments. Referred by a family doctor, contracted patients can make an appointment in the form of a priority to go to a higher hospital for specialist treatment. Higher-level hospitals open a certain number of number sources to family doctors.
3. Long prescription for chronic diseases. Family physicians can provide long prescriptions for treatment for contracted chronic patients, reducing the number of times patients go to a medical institution to prescribe medications. The Medical Insurance Fund reimburses long prescriptions in accordance with relevant regulations.
4. Referral green channel. Depending on the condition of the contracted patient, the family doctor helps him to be referred to a higher hospital for hospitalization. Higher-level hospitals provide referral facilities to contracted residents.

Family physicians provide a health examination once a year for people with essential hypertension and diabetes, including routine physical examination and health assessment. Fasting blood glucose tests are provided 4 times a year for diabetics. Follow-up is provided 4 times a year for patients with essential hypertension, diabetes, and patients with severe mental disorders who are clearly diagnosed and living at home. Patients with tuberculosis who are diagnosed and living at home are followed up once a month.
6. Child health management. Family physicians provide health management for children aged 0-6 years, including neonatal home visits, children's physical examinations, psychobehavioral development assessments, and TCM health guidance.
7. Maternal health management. Family doctors provide maternal health management, including the establishment of a Maternal and Child Health Manual, health assessment, first prenatal visits, pregnancy health education and guidance, and postpartum visits.
8. Health management of the elderly. Family doctors provide a health examination for the elderly aged 65 and over once a year, including routine physical examination and health assessment. Health guidance such as lifestyle, TCM physique identification and TCM health care.
9. Vaccination. Family doctors vaccinate children aged 0-6 years, vaccinate high-risk groups in key areas, and help manage vaccination information.
10. Health education. Family doctors provide contracted residents with health education knowledge such as healthy lifestyles, interventional risk factors, and prevention of infectious diseases. Is the contract service expensive?
How to charge for contracted service packages is divided into two types: basic packages and personalized service packages. Contracted service recipients are charged a contracted service fee on an annual basis, and the cost is shared by the basic public health service funds, medical insurance funds, and contracted residents in a certain proportion. The basic package reflects the public welfare, the charging standard is low, and the specific charging standard is determined by the local government (the low-security and other difficult groups are free). The personalized service package is based on the individual needs of residents, after deducting the proportion of other financing channels such as basic medical insurance funds or commercial health insurance, the remaining part is paid by residents.

The contracted service is based on the principle of voluntariness, and residents with the intention to sign the contract and the service needs can go to the nearby township health center, village clinic and other grass-roots health service institutions to choose a family doctor (team) to sign a service agreement, which covers the content, method, duration of the contracted service and the responsibilities, rights, obligations and other matters of both parties. In principle, the contract signing period is one year, and after the expiration of the period, residents can renew their contracts or choose other family doctors (teams) to sign up.
