Surgery is difficult in elderly patients

At the beginning of 2021, ms. In August 2021, Ms. Zhu suddenly had chest and back pain, and when she turned over and got up, she would have a painful attack, and then went to the First People's Hospital of Shaoguan City for treatment.

After diagnosis, Ms. Zhu suffered from osteoporosis with thoracic and lumbar compression fractures, old right femur intertrochanteric fractures with bone nonunion and bone non-concatenation, as well as severe osteoporosis and prolonged coagulation time.

Ou Guangxin, the doctor in charge, said that the difficulty of fracture surgery itself is not large, but the common underlying diseases of the elderly are prone to related complications during the operation period, even if there is no related basic disease, the physical function and function of the organs of the elderly patients are declined, and the tolerance to surgery and anesthesia and the ability to resist blows are weakened, and it is also easy to have related complications.

At the same time, elderly patients often choose lumbar anesthesia, which is commonly known as "semi-anesthesia", which does not require endotracheal intubation and has less impact on the patient's lungs. However, the patient has prolonged clotting time, is prone to bleeding, and cannot be anesthetized for lumbar stiff anesthesia due to concerns about epidural hematoma, and prolonged clotting time, which can also lead to an increased risk of intraoperative and postoperative bleeding. Elderly people are afraid to rehydrate too much due to concerns about dirty loads, and are prone to hemorrhagic shock.

Multidisciplinary collaboration

Considering the patient's advanced age, combined with the wishes of the patient and his family, after discussion in the department, the patient's surgery was carried out in two stages: first thoracic 12 and lumbar 1 percutaneous vertebral phylloplasty (completed under local anesthesia); the second stage was then considered to perform right hemixaphritis replacement.
