General consultation, finalization of the plan
How to help Cai Ren realize the "dream of making ears", and which treatment method is more suitable for him? On the morning of December 30th, during the hospital-wide general consultation organized by the Medical Department of CUHK Hospital, the doctors reviewed and inquired about Cairen's situation and made suggestions to discuss the best treatment plan together.
"The patient's loss of the right outer pinna is congenital microtia, which belongs to the 3rd degree microtia. Depending on the patient's situation and the shortest possible treatment time, it was decided to perform external ear reconstruction surgery for the patient. Professor Leng Yongcheng, a well-known burn plastic surgery expert and 90 years old, suggested after preliminary examination.
Wang Lei, director of the Department of Plastic Surgery of CUHK Hospital, introduced that the external ear reconstruction surgery performed for CaiRen should be divided into three stages. After completing the admission examination, a phase of surgery was performed on January 2, 2020, using the direct burial method, the rib cartilage was implanted behind the ear, and the ear of the affected side was carved according to the normal ear. The first stage is about 20 days hospitalized, and the second stage of surgery is performed half a year later. The second stage of surgery, by taking the skin in the chest, implanting rib cartilage again, etc., adds a "base" to the "lying ear" to make it "erect", and the ear is basically formed after the second stage of surgery. Three stages of surgery, re-trimming the ear. After about a year or so, after three stages of surgery, Cai Ren will have a pair of ears that are basically similar and symmetrical.
Work together to help realize your dreams
The appearance of the ear problem is solved, how to repair the hearing? Feng Xu, deputy director of the Department of Otolaryngology-Head and Neck Surgery of the hospital, said that after examining Cairen's ear, he found that "the patient's right ear does not have an external ear canal, but because the left ear is normal, it has not had a great impact on his language development." It is recommended to pass CT examinations such as CT after admission, check the internal development of the ear, conduct a hearing assessment, and check whether there is residual hearing in the right ear. After external ear reconstruction surgery, external ear canal plasty is considered. ”
Since Cai Ren is only 13 years old, Chief Physician Qiao Lixing, director of pediatrics, also participated in the consultation, and he suggested that "pediatricians will also assess the patient's psychosomatic development, and in the process of treatment, cooperate with plastic surgeons to guide medication." During hospitalization, the Department of Clinical Nutrition conducts a monthly nutritional assessment of Cai Ren. The anesthesiology department will also manage it in advance, do its best to protect the driver during the operation, and formulate a postoperative analgesia plan. The psychiatrist assesses Cai Ren's psychological condition, and then conducts psychological counseling for his inferiority complex.
Huang Yingzi, vice president of CUHK Hospital, said in the consultation that the hospital will fully support the establishment of the Dongzhou Cairen Treatment Group and work together for Cairen's "ear-making dream" in multiple disciplines.
Love is strong and warms people's hearts
In order to help Cai Ren realize his dream, not only did experts from various disciplines formulate the best treatment, but the medical staff also made meticulous arrangements for his life. Not only did he specially arrange a single ward for him to facilitate his grandmother's care. Three meals a day, all pay special attention to the canteen to prepare carefully, "they do not eat fish and shrimp, Grandma does not eat eggs, it is best not to have these in the three-meal menu." Lu Ling, the head nurse of plastic surgery, instructed the canteen manager one by one. On the coffee table in the ward, the medical staff also prepared small snacks for Cairen.
Since Grandma did not speak Chinese, the hospital also specially arranged 2 Tibetan medical students as volunteers. Grandma and the volunteers communicated in Tibetan, which also made her feel the warmth of home. Volunteers also prepared daily necessities for CaiRen and brought hot water bottles and soap.
I heard that there was a little Tibetan brother in the ward, and the little patients who were reconstructing their small ears in the ward sent greetings one after another to be friends with him. Cai Ren finally revealed a long-lost smile and tried to integrate into the warm family of the "Little Ear Home" of CUHK Hospital.
