Hu Bijie, an expert in the new crown treatment group: Asymptomatic infected people basically do not develop severe disease

From March 1 to March 6, in six days, Shanghai has reported a total of 12 local confirmed cases of new crown pneumonia and 71 asymptomatic infected people.

Wu Jinglei, director of the Shanghai Municipal Health Commission, said at the Shanghai New Crown Pneumonia Epidemic Prevention and Control Press Conference on the afternoon of the 6th that according to the relevant national standards, once asymptomatic infected person is diagnosed, it is necessary to undergo 14 days of isolation medical observation, and two consecutive nucleic acid tests are negative before the isolation medical observation can be lifted. After that, 14 days of quarantine health observation will be implemented in accordance with the relevant requirements.

How exactly do asymptomatic infected people undergo isolation medical observation? Will it progress to severe illness? The first financial reporter interviewed Hu Bijie, a member of the national medical treatment expert group for novel coronavirus pneumonia and director of the Department of Infectious Diseases and Infection Management of Zhongshan Hospital affiliated to Fudan University, on relevant issues.

CBN: At present, under the current round of new crown pneumonia in Shanghai (since March 1), mild patients and asymptomatic infected people have been transported to the Shanghai Public Health Clinical Center through a closed loop. Among them, what measures will be taken against asymptomatic infected people, and what is their current physical condition?

Hu Bijie: Asymptomatic infected persons, that is, those who have been infected with the new crown virus, the new crown virus nucleic acid test is positive, but there are no related symptoms, and there are no characteristic changes in lung imaging.

For asymptomatic infected people, they have neither symptoms nor clinical image changes, so we routinely take an isolated medical observation approach to asymptomatic infected people, and even this cannot be called "treatment". But most importantly, beware of further development of ordinary pneumonia in asymptomatic infected people, or the development of severe cases of COVID-19.

In the process of "medical isolation" for asymptomatic infected people, it is necessary to regularly carry out body temperature, blood monitoring and nucleic acid sampling, and at the same time, to ensure that asymptomatic infected people have a certain good sleep time, normal diet, and relaxation of the spirit to maintain a good state of immune function.

We pay special attention to the physical condition of the elderly, children and asymptomatic infected people with underlying medical conditions. Because the elderly often have diabetes, coronary heart disease and other underlying diseases, it is necessary to find problems through close observation of various indicators such as blood and organs, and timely targeted treatment to maintain their organ function and resistance.

Overall, judging from the situation in recent months, asymptomatic infected people are basically not going to develop severe disease.

CBN: In addition to asymptomatic infected people, what are the treatment methods for patients with mild diseases?

Hu Bijie: For mild and ordinary patients, we take the treatment plan of "three elements and one peptide". "One peptide" is the injection of thymus peptide, some elderly or poor immune function patients, after getting the new crown pneumonia, the disease is easy to progress, so subcutaneous injection of thymus peptide can be considered, for improving immune function, preventing the severeization of the disease, shortening the detoxification time, has a certain effect.

"Three elements" refer to heparin, hormones and interferons. Some patients with COVID-19 are more likely to develop blood clotting, and the use of low-molecular-weight heparin needs to be considered, while if patients with COVID-19 continue to have fever and pneumonia progression, short-term low-dose glucocorticoids may be considered. During this period, antibiotics must not be used blindly.

In addition to Western medicine treatment, in traditional Chinese medicine, it will also be used in the treatment of different types of patients, and different prescriptions will be prescribed for the different situations of each patient, that is, "one person, one post".

From prevention to the treatment of mild diseases, the treatment of severe diseases and the whole process of rehabilitation, we always adhere to the principle of combining traditional Chinese and Western medicine and attaching equal importance to Traditional Chinese and Western medicine, emphasizing dialectical treatment, one person and one party, and comprehensive treatment.

CBN: You mentioned earlier this year that for some patients with long hospital stays, the expert group gives nasal irrigation of iodine-containing drugs according to the situation, which can eliminate residual viruses in the respiratory tract, help accelerate patient detoxification, and promote recovery. Is this treatment just as effective for current mild patients?

Hu Bijie: The vast majority of patients turned negative for the virus after receiving treatment (about 2 weeks). However, there are also some patients whose virus has not disappeared after 3 to 4 weeks, and we found that part of the reason is that patients have paranasal sinusitis, and through nasal irrigation with a disinfectant, patients can speed up detoxification and promote recovery.

CBN: What will be the clinical changes in mild patients and asymptomatic infected patients?

Hu Bijie: The virus strain of this round of indigenous epidemic is the Aomi Kerong variant. This variant strain of pathogen has its own changes, overall, the pathogenic rate decreased, the proportion of lower respiratory tract infections in infected people decreased significantly, the proportion of fever decreased, and some infected people had no headaches and brain fever symptoms.

CBN: What is the situation of cured and discharged from Shanghai Public Health Clinical Center?

Hu Bijie: After the implementation of the individualized diagnosis and treatment plan, our expert group made collective rounds every day, studied and judged the condition of each patient, adjusted the diagnosis and treatment measures, and strengthened humanized services, especially psychological services, dietary services, and so on.

In this process, we focus on the elderly and children, as well as those infected with underlying diseases, closely observe the changes in the condition, assess possible risks, actively prevent complications, and prevent mild diseases from turning into severe diseases.

After careful treatment and care by the Shanghai Public Health Clinical Center and other municipal hospitals, and after the evaluation of the municipal expert group, today, 65 confirmed cases of new crown pneumonia have been discharged from the hospital and 16 asymptomatic infected people have been released from medical isolation and observation, a total of 81 people.
