The patient was almost killed by stomach discomfort, and Bao'an First Hospital successfully rescued him with an ECMO artifact

Recently, 47-year-old Ms. Yang's stomach was swollen and uncomfortable, and she could not be treated for a long time, resulting in serious illness and imminent death. Shenzhen Hospital of Integrated Traditional Chinese and Western Medicine used the artificial membrane lung (ECMO) artifact to save it and give it a new life.

It is understood that Ms. Yang usually feels that the stomach is swollen and uncomfortable, and each time she takes stomach medicine to alleviate it slightly. Recently, the symptoms have occurred frequently but they have not paid attention to it, thinking that the annual physical examination is normal, the problem is not big, and the patience has passed.

Some time ago, Ms. Yang felt discomfort in the stomach and low-grade fever, thought she was cold, and did not care. The next day, the symptoms still did not improve, fatigue, dizziness, no appetite after going to Shenzhen Integrated Traditional Chinese and Western Medicine Hospital to see a doctor.

After investigation, Ms. Yang's troponin was 100 times higher than normal, and her ECG showed left bundle branch block, ventricular early and short bursts of "ventricular rate", indicating heart damage and serious condition, that is, hospitalization.

After reviewing Fang Hongcheng, director of the Cardiovascular Department of the hospital, he believed that the patient had no history of heart disease, signs of heart and kidney failure and sudden severe arrhythmias, and considered that acute myocarditis was likely, that is, supportive treatment and absolute bed rest.

In the early morning of the next day, the patient's heart function continued to deteriorate, followed by fatigue, nausea and vomiting, cold sweating all over the body, and restlessness. Bedside blood pressure as low as 70/40 mmHg, significant symptoms of shock, and rapid referral to the Intensive Care Unit (ICU) for treatment.

After assessing the condition, ICU Director Lai Jianbo considered that the possibility of fulminant myocarditis was high, and once severe shock, the mortality rate was as high as 45%, and the treatment pipeline was laid immediately, and the V-A ECMO technology was activated.

Unexpectedly, during the disinfection preparation, the patient suddenly had a cardiac arrest, and most of the peripheral blood vessels in the whole body were in a collapsed state, which suddenly increased the difficulty of catheterization, and could only perform cardiopulmonary resuscitation with bare hands while exploring the puncture catheter.

After 2 hours, the ECMO pipeline was successfully placed and the treatment was run, and the patient's vital signs and cardiac function gradually stabilized. Due to cardiac arrest and shock, the patient's liver, kidney, pancreas and other organs are severely failed.

In order to ensure strong organ function support, renal replacement therapy and artificial liver therapy (bilirubin adsorption + continuous blood filtration) have been added successively, and then the patient's multi-organ function has gradually improved, saving his life.

On 29 March, the patient was awake, his vital signs were basically stable, he was intubated and gradually recovered. On 10 April, vital signs were stable and he was transferred out of the ICU to the general ward for further consolidation.
