A pair of twin brothers were born prematurely, thin and small, the weight came out of the mother's belly, and they faced a life crisis just 48 hours after birth. The brothers and brothers had bloating in their stomachs one after another, and the examination found that it was caused by a "leak" in the stomach. Two little lives are at stake. The "Guardian Newborn" medical transport team of Wuhan Children's Hospital brought them back to Wuhan, and neonatal surgery experts relayed emergency surgery. Fortunately, the operation was timely, and after nearly half a month of treatment, the brothers can now breastfeed and gain weight. You can recover and be discharged from the hospital at the weekend.

Premature male babies have gas leaks from their stomachs

"There was a pair of low-weight premature babies, suspected of having a perforated digestive tract, in critical condition, and requested to be transferred." On the afternoon of March 5, Wuhan Children's Hospital received a transfer application from a doctor from Hubei Pediatric Medical Alliance Hospital and Jingmen Traditional Chinese Medicine Hospital.

In desperate need of transport are twin brothers, just two days after their birth. They spent only 32 weeks in their mother's belly, and both were born low-weight premature babies, with an older brother of 1.84 kilograms and a younger brother of 2.1 kilograms.

In contrast, the brother's condition is more serious, vomiting after breastfeeding, there is also bloating, blood in the stool, the local hospital film to see, the abdominal cavity is full of gas, the initial diagnosis of digestive perforation. If surgery is not possible in time, digestive juices and feces reach the abdominal cavity, which can cause peritonitis and endanger life.

Young lives are at stake and surgery is necessary as soon as possible. After receiving the transfer application, the medical transfer team of Wuhan Children's Hospital "Guarding the Newborn" immediately assembled and set off, and the neonatal physician Wang Huizhen and nurse Wang Bei traveled nearly 500 kilometers, round and round, back and forth for 6 hours, bringing the brothers back to Wuhan safely.

Relay surgery to save your baby

After arriving at Wuhan Children's Hospital, my brother took the "green channel" and was sent to the Neonatal Surgery Department to prepare for emergency surgery.

"When I first arrived, the child was short of breath, his small belly was bulging very high, the local abdominal wall was red, and his response was extremely poor." Xiong Xiaofeng, the chief surgeon and neonatal surgeon of the hospital, recalled that the baby was at high risk, combined with gastrointestinal perforation, and there was a manifestation of diffuse peritonitis, and intra-abdominal hypertension affected the child's respiratory and circulatory function, and cardiac arrest may occur at any time, or lose his life due to infectious shock.

Doctors first urgently give celiac puncture to pump air, replenish blood volume, and quickly improve preoperative preparations. At 9:45 p.m. that night, less than an hour after admission, my brother was sent to the operating room, and the neonatal surgery department and the anesthesiology department cooperated to operate on the baby and determine where the crack was as soon as possible.

During the operation, under laparoscopic exploration, the doctor determined that it was a congenital gastric wall muscle layer defect and gastric rupture. That is to say, in the anterior wall of the stomach near the cardia, there is a rupture 5 cm long and 3 cm wide. Doctors surgically cut the broken stomach wall tissue and repair the remaining stomach wall. Within 2 hours, the child's ruptured stomach was repaired. At the same time, the younger brother also found the same problem and also surgically solved the rupture.

The neonatal surgeon reviews the twins after surgery

The first 3 to 5 days of life is a high incidence period

After more than 20 days of treatment and care, the twin brothers survived important life-threatening obstacles such as respiratory dysfunction, stomach fistula, and severe infection, and now the two have begun to eat and gain weight. Knowing that the twins were not wealthy, the hospital's medical social worker and the department's charity commissioner applied for charitable assistance for them, helping to fill the cost gap.

Why do both babies have stomach ruptures? Lu Wei, deputy chief physician of the Neonatal Surgery Department of the hospital, said that the clinical rupture of the stomach in newborns is rare, and it is rare for twins to have stomach rupture at the same time, and there is only one case of literature reports that can be found in China. According to the analysis, this is related to the fact that they are premature babies with poor muscle development of the stomach wall.

Lu Wei said that the rupture of the stomach in newborns is one of the most serious diseases in pediatric surgical acute abdomen, and the first 3 to 5 days after birth is a high incidence period, which is initially manifested as refusal to eat, vomiting, mental malaise, etc., followed by progressive bloating, dyspnea, cyanosis, vomiting aggravation with blood or coffee-like, blood in the stool. Acute onset, rapid development, can cause severe respiratory disorders and septic shock, is a high mortality rate of pediatric surgical diseases, once diagnosed should be emergency surgery as soon as possible.
