Usually, a woman's first menstrual period is at the age of 12-15 years, which is the most important feature of the beginning of ovarian development, initial ovulation and endocrine function. In the following decades, the ovaries will undergo regular ovulation until the woman enters the age group of 45-55 years old, and the woman will gradually usher in menopause due to the aging of ovarian function.
In China, the vast majority of women are actually completely menopausal before the age of 50. In other words, menopause after the age of 40 and before the age of 50 is normal. However, this is not completely fixed, some women are affected by genes, menarche too late, menopause will also be delayed, before the age of 60 menopause is also more common.
However, if a woman has not yet entered menopause, especially between the ages of 40, it is not a normal manifestation, but may be related to factors such as disease and premature ovarian failure, which needs to be paid attention to.
So the question is, women have not yet reached the menopause period of amenorrhea, how to treat it?
1. Treatment of etiology
If you want to regulate amenorrhea, you must first find the cause of amenorrhea and carry out targeted treatment. For example, nerve and mental stress factors should be caused by effective psychological counseling after all.
However, due to poor physique, excessive dieting, abnormal wasting, obesity, etc., dietary adjustments should be made, starting from nutrition, and amenorrhea caused by hypothalamic, pituitary tumors, and ovarian tumors need to be removed through surgery. Usually after the cause is eliminated, menstruation returns to normal;
2. Endocrine therapy
According to the etiology, pathological characteristics, physiological mechanisms and other considerations of amenorrhea, some patients need to take targeted endocrine drug therapy in order to correct the hormone levels of disorders in the body, and then achieve the purpose of treatment of menstrual cramps.
Patients with CAH, for example, require long-term treatment with glucocorticoids. Patients with PCOS with significant hyperandrogenemia require estrogen-progestin-plus oral contraceptive therapy. For patients with PCOS who already have insulin resistance, insulin sensitizer therapy may also be an option. Through these treatments, patients' menstruation will gradually recover, and some patients' ovulation function will return to normal;
3. Hormone therapy
After all, the first treatment used in adults due to hypoestrogenemia is hormone administration. Usually, the clinic will recommend that patients use 17β-estradiol or acid estradiol, or in combination with estrogen therapy, to promote and maintain general health and sexual development.
After the uterus develops, it is necessary to regularly add progesterone and estrogen progesterone according to the degree of endometrial proliferation. If the patient also has androgen excess, a combination of progestin formulations containing antiandrogen effects may be used. For people with a certain level of endogenous androgens, progestin treatment should also be used regularly, only in this way can the endometrium be shed regularly.
In general, if a woman has amenorrhea before menopause, patients should be vigilant and seek medical attention for relevant screening at the first time, including sex hormone level examination, thyroid function test, uterine and ovarian ultrasound examination, and so on. Only after finding the cause of amenorrhea can clinically be targeted treatment.
