In order to deeply learn the lessons of the safety production accident of limited space operation, conscientiously implement the relevant instructions and instructions of provincial, municipal and district leaders, and earnestly grasp the safety production of limited space in the field of construction engineering in the city. In accordance with the "Notice on Printing and Distributing the Implementation Plan for Further Strengthening the Risk Prevention and Control of Confined Space Operations in Chengdu" (Chengdu Safety Committee [2023] No. 8) and the "Emergency Notice on Immediately Carrying out the Special Investigation and Rectification of Safety Hazards in Confined Space Operations" (Cheng An Ban [2024] No. 30), the District Housing and Urban-Rural Development Bureau has decided to carry out special rectification of the management of confined space operations in the field of construction engineering in the whole district, and this work plan is hereby formulated.

1. General requirements

In-depth study and implementation of General Secretary Xi Jinping's important expositions on production safety, and fully implement the Party Central Committee, the State Council, the provincial party committee, the provincial government, the municipal party committee and the municipal government, The requirements of the district party committee and district government on the deployment of safety production work, combined with the special rectification of limited space operation management, comprehensively improve the safety prevention and control level of limited space operations, strengthen risk prevention and control and hidden danger investigation and management in the field of construction engineering, improve the safety and long-term management mechanism, severely crack down on all kinds of violations of laws and regulations, effectively prevent and curb limited space production safety accidents, and ensure that the city's safety production situation is stable and improving.

Second, the scope of remediation

(1) Whether there is any of the following limited spaces. Limited space for housing construction projects: civil air defense engineering, artificial digging pile engineering, steel box girder, fire pool, pumping station, elevator shaft, ventilation shaft, lighting well, storage room, underground parking lot, underground engineering of residential property, etc.; limited space for municipal infrastructure engineering: underground pipe gallery, tunnel, construction shaft, rain and sewage well, power well, heat well, telecommunication well, gas well, water collection well, sewage tank, biogas tank, septic tank, pipe trench, mixing equipment, various storage tanks, etc.

(2) Whether there are any of the following hazardous substances in the confined space. Building materials: concrete additives, waterproof coatings, anti-corrosion and thermal insulation materials, volatile organic solvents, and other construction materials containing benzene, toluene, xylene, ammonia, polyurethane and other substances. Hazardous substances present in or produced in the construction environment: coal, gasoline and diesel combustibles, carbon monoxide, carbon dioxide, sulfur dioxide, hydrogen sulfide, dust, gas, etc.

(3) Whether there are any of the following dangerous operations in the limited space. Waterproof construction, underground excavation construction, pipe jacking construction, shield construction, mold removal operation, electrical welding operation, paint spraying operation, anti-corrosion and thermal insulation operation, winter open flame insulation construction, manual digging pile operation, all kinds of tube well maintenance, cleaning and upgrading operations, dredging operations, internal combustion engine (water pumps, gasoline and diesel generators, etc.) operations, etc.

III. Emphasis on Healing

(1) Carry out the identification of limited space. The construction and supervision units of each project should comprehensively sort out the limited space situation, determine the basic situation such as quantity, location and dangerous and harmful factors, find out the base number of limited space and the current status of safety management, establish and improve the limited space ledger and update it dynamically. At the same time, it is necessary to strengthen the docking with all kinds of pipeline ownership units, find out the situation of various pipelines, shafts and inspection wells in the construction area and adjacent areas, and avoid damaging the sewage, gas and other pipe networks that cross in parallel.

(2) Prepare a special construction plan. If the identified hidden dangers in the confined space cannot be rectified immediately, before the operation is carried out, the operating environment shall be assessed, the existing dangerous and harmful factors shall be analyzed, measures to eliminate and control the hazards shall be proposed, and a confined space operation plan shall be formulated and approved in accordance with the regulations. Before the limited space operation, the contractor engaged in the specific construction operation of the limited space (labor, professional subcontractor, etc.) must report to the project construction, supervision, and construction unit for approval, and there must be a guardian during the operation to continuously monitor the whole process.

(3) Strengthen operational protection and supervision. Strictly implement the principle of "ventilation first, then testing, and then operation". Ventilation measures must be considered before and during the operation of confined space, especially in places where the risk of poisoning and suffocation occurs such as biogas digesters and sewage wells, and equipment such as blowers should be used for forced ventilation. During the ventilation process, gas detectors and other equipment are used to detect the gas concentration in the limited space to ensure that the concentration of oxygen, harmful gases, combustible gases and dust meets the safety requirements before entering the operation. Equipped with lighting, communication, emergency rescue and other operating equipment and personal protective equipment that meets national standards and industry standards required for all confined space operations. Operators should be tied to a safety leash for operation, and at the same time, on-site supervisors should be equipped to supervise the whole process, and should avoid the high temperature period during the day for limited space operations to prevent accidents. After the operation is completed, the person in charge of the site and the guardian should clean up the operation site and evacuate.

(4) Strengthen safety training for employees. The construction unit shall organize the relevant production and business operation unit responsible persons, safety management personnel, on-site responsible personnel, guardians, operators, and emergency rescue personnel to carry out warning education and training on a regular basis, focusing on training the type, quantity, distribution, and risk factors of limited space, limited space operating procedures, operating procedures, and operation protection requirements, and the correct use of relevant equipment, testing instruments, and labor protection equipment, and emergency measures in case of emergency. At least 1 training per year, and 1 safety notification and pre-job training before the implementation of the operation. All personnel must go through strict education and training, and master the knowledge and ability of confined space operation before they can work. For production and business operation units that do not have training capabilities and conditions, they must entrust professional and technical service institutions or hire experts to carry out education and training, and the training materials shall be archived for not less than 1 year. Strengthen the management and maintenance of equipment and facilities, and designate a special person to establish an equipment ledger, responsible for maintenance, maintenance and regular inspection and calibration, etc., to ensure that it is in good condition, and when it is found that equipment and facilities affect the safe use, they should be repaired or replaced in time.

(5) Formulate emergency rescue plans. The relevant production and business operation units shall, according to the characteristics of confined space operations, identify possible safety risks, clarify the division of labor and responsibilities of rescue work, on-site disposal procedures, etc., formulate scientific, reasonable, feasible and effective special emergency plans or on-site disposal plans for safety accidents in confined space operations, and organize regular training to ensure that the person in charge of the confined space operation site, guardians, operators and emergency rescue personnel grasp the content of the emergency plan. The special emergency plan for safety accidents in confined space operations should be organized at least once a year, and the on-site disposal plan should be organized at least once every six months. In the process of limited space operation, the person in charge of the site should organize and command the whole process, and the guardians should supervise the implementation of the operation plan throughout the process.
