Tumor patients fainted at home, it was "cancer plus cancer"

Ms. Wang, 57, of Changsha, Hunan Province, has been feeling tired since August last year, often feeling weak and dizzy, and has fainted at home twice. After fainting in September 2021, he was sent to a nearby hospital by his family, and the blood routine found that the white blood cells were very high, reaching 120×10^9/L (the normal value of white blood cells was 4-10×10^9/L), and was accompanied by severe anemia and severe thrombocytopenia, in which case Ms. Wang was at risk of major bleeding at any time.

The doctor informed the seriousness of the condition, and Ms. Wang and her family immediately went to the oncology department of the Second People's Hospital of Hunan Province (Hunan Provincial Brain Hospital), and Zhu Zhongshan, deputy chief physician, received the treatment. After reviewing her blood routine report, she immediately contacted the ward doctor to prepare for blood transfusion treatment, and Ms. Wang was highly suspected of having acute leukemia.

Dr. Zhu continued to inquire about Ms. Wang's condition and past medical history, and asked Ms. Wang whether her home had been renovated recently, whether she had been engaged in work exposed to radioactive substances, or had been exposed to toxic chemicals. Asked if there was any other medical history before, Ms. Wang immediately said: "Oh, I had breast cancer at the end of 2016, the right breast, at that time I had surgery, and I also did radiotherapy and chemotherapy, the treatment effect was very good, last year I also did a comprehensive examination without recurrence, and my body recovered quite well, almost 6 years." The family also said next to them: "Yes, the previous breast cancer recovered very well, doctor, is this breast cancer recurrence and metastasis?" ”

Dr. Zhu Zhongshan explained: "First of all, Ms. Wang found that the white blood cells were very high in this examination, suspecting that it was leukemia, and the next step was to do a bone marrow puncture examination to confirm the diagnosis. Ms. Wang's current situation is considered to be the second primary tumor, that is, the emergence of a new malignant tumor on the basis of the previous breast cancer."

After being admitted to the hospital, Ms. Wang's perfect bone marrow examination clearly diagnosed as "acute leukemia", after blood transfusion treatment, dizziness and fatigue have been alleviated, and further treatment is currently being carried out for leukemia.

The patient's family secretly found the doctor: "How can my lover get this disease again, breast cancer is not easy to cure." "Dr. Zhu Zhongshan comforted the family, now there are data surveys showing that 8% of adult tumor patients may suffer from the second primary tumor, the most common secondary primary tumor in breast cancer patients is contralateral breast cancer, followed by malignant tumors of the respiratory tract, digestive tract and female reproductive system, including hematologic malignancies.

The fatality rate of second tumors is often higher, with more than half of patients dying from second primary tumors. For patients who survive for a long time after tumor treatment, we should attach great importance to the occurrence of the second primary tumor, urge patients to follow up and review regularly, strengthen the monitoring of various indicators of the body, and if a second tumor occurs, it is also necessary to achieve early detection, early diagnosis, and early treatment.
