The disease itself does not affect the patient's fertility, so it cannot be killed with a stick, and it is determined that female patients with SLE cannot get pregnant normally. But there are more places for SLE patients to be aware of when getting pregnant than the average woman. For example, some drugs for the treatment of SLE, such as cyclophosphamide, triptolide, etc., will indeed affect women's fertility; blindly stopping drugs in order to get pregnant is more likely to lead to adverse outcomes such as miscarriage and stillbirth.

What do people with SLE need to prepare if they want to get pregnant?

First of all, we must face up to the disease and actively treat it! Control the disease in a relatively stable quiescent period (usually more than 6 months). Some expectant mothers will be more anxious, SLE has not been well controlled to start trying to conceive, once pregnant, it is easy to worsen SLE and induce complications (such as miscarriage, premature birth, eclampsia, fetal SLE, etc.), exacerbating the risk of pregnancy of expectant mothers, and even life-threatening. Therefore, in addition to the basic preconception examination, SLE patients who want to become pregnant should also go to a specialist to assess the disease state at the time of pregnancy, whether there is damage to important organs, such as kidneys (the third trimester will increase the burden on the kidneys, accelerate renal dysfunction, and even lead to kidney failure), if lupus nephritis has been diagnosed, it is recommended to postpone pregnancy and control renal function to a reasonable range before trying to conceive.

Secondly, if the drug cannot be completely stopped before pregnancy, you must find a specialist to adjust the drug, choose a drug that is relatively safe during pregnancy, and do not stop the drug as soon as you find out that you are pregnant. What are the drugs that are relatively safe for patients with SLE during pregnancy?

If SLE expectant mothers need to take medication during pregnancy, the drug with the highest current safety is hydroxychloroquine, followed by low-dose aspirin. The effects of hydroxychloroquine on mothers, mainly ototoxicity, retinal bleeding, or pigmentation, require active monitoring and prevention. For the fetus, although it is still shown in animal experiments that hydroxychloroquine will pass through the placenta, in the current limited clinical data, it cannot be shown that hydroxychloroquine directly causes fetal dysplasia. The most common effect of aspirin is bleeding, which can also lead to anemia in expectant mothers, prenatal or postnatal haemorrhage, pregnancy or prolonged labour. However, there are also no data to show a direct correlation between low-dose aspirin and fetal malformations. It must be emphasized that whether to use drugs or not, and which kind of drugs to use, need to be evaluated by specialists and pharmacists.
