Little Dolphin Class|The baby has repeated urinary incontinence, and it turns out that this disease is at fault

author:Guangxi Maternal and Child Health Hospital

5-and-a-half-year-old Satsuki (pseudonym) often wets his pants during the day because he often does not involuntarily urinate during the day, but he sleeps at night and has no signs of urinary incontinence. I went to the hospital for examination, and there was no obvious abnormality in the improvement of urinary system B ultrasound and urine routine in the outer hospital, and after observing for a period of time, there was still a manifestation of wet pants during the day, which seriously affected Xiaoyue's quality of life~~~

So, with apprehension, the parents came to the Guangxi Zhuang Autonomous Region Maternal and Child Health Hospital for treatment, and the outpatient doctor recommended hospitalization after careful medical history, and improved the urinary magnetic resonance (MRI) examination, which showed repeated kidney repeated ureters and hydronephrosis.

After hospitalization, the doctor performed minimally invasive surgery on Satsuki with laparoscopy, and the postoperative urine leakage disappeared, and the hydronephrosis improved significantly, and she has been successfully discharged from the hospital.

Younger children may not have the habit of controlling urination, but if the child has been suffering from urinary incontinence since childhood, and urinary incontinence occurs between two normal urinations, we should highly suspect that the child has the possibility of repeated kidney malformation.

Popular science on maternal and child health

What is a duplicate kidney?

Duplex kidney is a more common congenital malformation of the kidney and ureter, with an incidence of about 1/1500, and the duplicate kidney is not an extra kidney, most of which are fused into one and cannot be separated, and there is a shallow groove on the surface, with their respective renal pelvis, ureters and blood vessels. Duplicate kidneys can be unilateral or bilateral, and duplicate kidneys and repetitive ureters are often present at the same time, and can open in the bladder or ectopic in the urethra, vestibule, or vagina.

What are the symptoms of duplicate kidneys?

1. Some repetitive ureteral malformations in which the ureters open in the bladder, such cases have no clinical symptoms at all, and are only detected during a comprehensive urological examination. Such patients account for about 60%. (See Figure 1)

Fig.1 Repeating the renal repeating ureteral opening in the normal bladder position

2. Repeated kidney may be accompanied by symptoms such as pyelitis, kidney stones, tuberculosis, tumors, and hydrops.

3. There are also some duplicate renal repeat ureteral malformations, and the ureters open in the vulva, vestibule, vagina, etc. As a result, the child has a history of enuresis since childhood, wetting the bed at night, and often wet pants during the day, but the patient has normal urination activities. If there is a history of this, careful examination of the vulva often reveals an abnormal ureteral opening. (See Figure 2)

Do duplicate kidneys need to be treated? What happens if the kidney is left untreated?

1. Patients with no clinical symptoms, no hydronephrosis and no renal impairment do not need surgical treatment;

2. Patients with mild symptoms may have vulvar skin eczema, itching and peculiar smell due to repeated wet pants, and may have psychological effects in adulthood; Patients with severe symptoms may have recurrent urinary tract infections, hydronephrosis, impaired kidney function, and even affect the function of the kidneys in the lower normal group. Therefore, symptomatic children should be actively examined and treated in a regular specialist hospital.

Repeat the surgical modus elegia

Laparoscopic minimally invasive surgery: repetitive kidney with clinical symptoms needs to be surgically treated, and the current use of laparoscopy for urinary system surgery has been very mature, it has the advantages of quick recovery after trauma, light pain, and beautiful surgical opening, etc., which has been accepted by the majority of doctors and patients' relatives, and has been carried out rapidly, and has gradually become a routine operation in pediatric urology.

Our department can routinely carry out laparoscopic minimally invasive surgery for a variety of urological diseases, and is one of the few hospitals in Guangxi that can perform pediatric laparoscopic urological surgery.
