Case Study (1)

author:China Maritime

If the mechanical and electrical equipment (main propulsion device, main power supply, emergency power supply, boiler, steering device) fails in the process of entering and leaving the port and channel with heavy water traffic, the ship's maneuvering will be limited, and it is very easy to cause dangerous accidents.

Since April 7, the Maritime Safety Administration of the Ministry of Transport has carried out a nationwide special action to prevent the failure of ship mechanical and electrical equipment, focusing on reducing the risk of ship mechanical and electrical equipment failure, and focusing on preventing and curbing water traffic accidents.

Case Study (1)

Some of the cases in the special action are representative and worthy of our reference.

Below, the editor introduces two related cases, analyzes the hidden dangers of the same type of main engine power system exposed by the cases, and hopes to arouse the vigilance and attention of the company and the ship, draw inferences from one another, and strengthen investigation and self-correction.

The same type caused by a near miss

Investigation and correction of hidden dangers

One

Near danger of failure of the main power system

In January 2024, during the departure of the "N" ship of a large bulk carrier from the port, the ship lost power and drifted due to a sudden main engine failure on the inner side of the port basin, and there was a risk of colliding with the light float, and the pilot reported to the maritime department as soon as possible. The local maritime authority immediately activated the emergency response, using tugboat towing to fix the ship's position, and after the ship was temporarily repaired and restored to power, it was safely moved to the anchorage for anchorage and maintenance under the escort of the tugboat.

After the port state supervision and inspection, it was found that the electromagnetic inductor fixtures of the three reversing cylinders No. 4, 5 and 6 of the main engine were loose and fell off, and when the main engine was reversing the operation, the control system could not receive the calibration electromagnetic signal fed back by the reversing cylinder, resulting in the ignition failure of the main engine and the loss of power of the ship.

Two

The same type of problems found in port state control inspections

In April 2024, the maritime affairs department carried out a special inspection on the "L" ship of a bulk carrier to prevent the failure of the ship's mechanical and electrical equipment in combination with the port state supervision and inspection. The model of the main engine is the same series as the "N" wheel, combined with the experience of the "N" round inspection, the law enforcement officers focused on the preventive inspection of the remote control reversing system of the main engine, and found that the buckle of the steering cylinder induction solenoid valve of the No. 4 cylinder of the main engine has been loosened, and there is a risk of the induction solenoid valve falling off.

The law enforcement officers will immediately feedback the defects to the responsible crew and carry out case publicity and education, and require thorough repair and testing before sailing to ensure that the main engine is kept in good condition during the ship's departure from the port.

Case Study (1)

The same type of case is exposed

Hidden dangers of the main power system

N wheel and L wheel main engine models are MAN B&W 6S70MC, different from the current popular ME EFI host, MC host is a traditional diesel engine using camshaft/cam drive for timing control, which is widely used in some older ships, this type of host adjusts the camshaft through the reversing cylinder and then controls the ship's entry and reverse operation.

In order to prevent the main engine from starting in the case that the camshaft is not in place (which may cause damage to the machine), the main engine of this type is provided with an electromagnetic inductor on the steering cylinder block, but the position of the reversing cylinder and the supporting electromagnetic inductor is relatively hidden, and the electromagnetic inductor failure and other situations can not be displayed separately on the main control panel, and the crew is easy to ignore in carrying out daily inspection and maintenance.

When the buckle of the induction solenoid valve of the steering cylinder is loose or falls off, it is not easy to be found, and the detachment of the induction solenoid valve will cause the control system to not receive the calibration electromagnetic signal of the reversing cylinder feedback when the main engine is reversing, and then lead to the ignition failure of the main engine and the shutdown.

Security management recommendations

In view of the same type of problems exposed by the above two cases, all shipping companies and crew members should reasonably formulate maintenance plans in combination with the instructions of the main engine, strengthen the control of the remote control reversing system of the main engine and the maintenance and inspection of components, especially the ships installed with the same type of main engine should strengthen the verification of the condition of the steering cylinder induction solenoid valve to avoid the failure of the main engine caused by the same type of problems.
