Recently, the Department of Neurosurgery, General Surgery and Interventional Surgery of Lanshan County Central Hospital successfully completed two cases of arterial embolization interventional surgery in close collaboration, one case was "right lower extremity angiography + balloon dilation + catheter thrombolysis", and the other was "splenic angiography + aneurysm embolization". The successful development of the two surgeries filled the gap of arterial thrombotic interventional surgery in Lanshan County, marking a new level in the hospital's arterial interventional diagnosis and treatment technology.

The first case of surgery was Uncle He, a 74-year-old who lived in the city and township, who went to the neurosurgery and general surgery department of the county central hospital for "1 week of progressive pain in the right lower limb". It is reported that Uncle He had limited movement of his right lower limb 2 years ago, and the resting pain was obvious. After being admitted to the hospital, Dr. Liao Junjie immediately examined him, and the arteriovenous vascular color ultrasound showed: thrombosis of the inferior part of the superficial femoral artery, popliteal artery and posterior tibia artery of the right lower limb; arteriosclerosis of the right lower extremity changed the acoustic image and the formation of plaques on the wall.

Due to his advanced age and wide range of thrombosis, if he adopts traditional incision and thrombotic treatment, the risk of surgery is extremely high, the risk of bleeding is extremely high, and the long period of bed rest will greatly increase the risk of surgical complications. In order to prevent Uncle He from irreversible ischemic necrosis of the lower limbs, the department team invited interventional experts to consult, and after careful assessment and full preparation, formulated a plan of "right lower extremity angiography + balloon dilation + catheter thrombolysis" for him, and at the same time fully informed the family of the possible risks and complications of the operation, and the operation was carried out as scheduled after obtaining the consent of the family.

After the operation, the blood circulation of the blood vessels in Uncle He's lesion was smooth, the artery of the right lower limb beat well, the skin temperature was greatly improved, the resting pain was significantly reduced, and he was successfully discharged from the hospital.

The second case of surgery was Mr. Xiao, a 46-year-old resident of Xinwei Town, who was admitted to the hospital for "four years of repeated left waist pain", and Dr. Li Xiaocong immediately examined him, and the CT showed a splenic aneurysm. This aneurysm is so harmful because of the loose surrounding tissues that once it ruptures, it is like a dam bursting its banks, and the patient will quickly lose blood and endanger life. At the same time, there are many blood supply arteries and complex structures for important organs around the aneurysm, and the traditional surgical open-abdominal clipping surgery is traumatic and high-risk. After several consultations, the experts decided to use minimally invasive interventions with less trauma. Because it is a mutated aneurysm, the interventional surgery is also more difficult, the tumor is large and the tumor wall is thinner, and it is easy to rupture and cause fatal bleeding, which puts forward higher requirements for the accuracy of operation.

In late January, under the premise of full preoperative preparation, experts carried out interventional surgery for Mr. Xiao. The operation is performed in the tumor cavity, where the doctor places a catheter into the tumor cavity and releases a controllable spring ring inside to seal the tumor. Relying on clear medical image evaluation and rich experience in interventional surgery, it took more than 2 hours to successfully complete the "aneurysm embolization". When the radiographer pressed the contrast button, the huge tumor cavity disappeared, and the surrounding blood vessels walked and the blood flowed as usual, and the operation was a complete success. Mr. Xiao recovered well after surgery, had no complications, and was successfully discharged from the hospital in early February.

The successful implementation of this procedure significantly shortened the patient's hospital stay and avoided the risk of splenectomy that may lead to collateral artery injury, splenomeric artery trunk injury and splenometry due to splenectomy when an aneurysm is removed by open surgery.
