The newborn suddenly cried incessantly and had sluggish eyes, which turned out to be a sudden cerebral hemorrhage

Recently, the 3-week-old Yue Yue (pseudonym) suddenly cried incessantly, her eyes were sluggish, and anxious parents rushed to the emergency department of Qingdao Women and Children's Hospital with their children in their arms. Combined with ancillary examinations such as brain CT, the doctor judged that Yue Yue suffered from bilateral intraventricular hemorrhage with ventricular dilation, and his life was in danger, and he must be rescued immediately.

The newborn suddenly cried incessantly and had sluggish eyes, which turned out to be a sudden cerebral hemorrhage

With the cooperation of NICU, pediatric anesthesiology and other departments, the neurosurgery team carried out symptomatic rescue overnight to remove intraventricular hemorrhage, and the child's condition was temporarily stabilized. However, Sun Yong, director of neurosurgery, believes that the alarm has not been lifted, there is no cause of common intraventricular hemorrhage in newborns such as premature birth, hypoxia, and abnormal coagulation function, and such a serious cerebral hemorrhage is likely to have other problems. In order to find out the cause, the neurosurgery team improved the relevant examinations such as MRI and found that there are still more cast blood clots in the intraventricular of the moon and moon, which is likely to be cerebral hemorrhage caused by rupture of vascular malformations.

Compared with adults, children have a lower incidence of cerebrovascular malformations and are more rare in the neonatal period, but are at relatively high risk of repeated bleeding and neurologic dysfunction. Surgery is imperative, because the child is young, low weight, small skull, weak brain tissue, and the lesion location is located in the central area near the brain, a little carelessness will cause bleeding and nerve function damage, so the operation is more difficult.

After thorough preoperative preparation, the neurosurgery team performed a microscopic surgical procedure for the moon and the ventricular vascular malformation. The intracranial blood vessels of the newborn are slender and easy to be injured by mistake, and Director Sun Yong avoids the functional area of the brain through the cerebral sulcus for corticotomy into the ventricle, carefully peels off the deformed blood vessels in the narrow fistula, and then completely removes them. It took 4.5 hours and the surgery was successfully completed. At present, he is recovering well and has been discharged from the hospital.

At present, Qingdao Women and Children's Hospital has successfully treated a number of critical intracranial hemorrhage newborns and premature infant cases, through multidisciplinary joint diagnosis and treatment (MDT), perioperative precision management, individualized and minimally invasive treatment of newborns and premature babies with cerebral hemorrhage and achieved good results, and has reached the domestic advanced and provincial leading level.

Neurosurgery experts from Qingdao Women and Children's Hospital remind that intracranial hemorrhage in newborns is generally related to factors such as ischemia and hypoxia, premature birth, birth injury, vascular malformation, abnormal coagulation function and maternal body. Intracranial hemorrhage may form a severe brain dysfunction, and early diagnosis and early treatment can effectively improve the prognosis. Newborns with intracranial hemorrhage are mostly convulsive, irritable, mentally weak, drowsy, milk refusal, pallor, vomiting, screaming, etc. If parents find that the baby has the above symptoms, they should see a doctor in time.
