Professor Yi Chenggang, director of plastic surgery at the Second Hospital of Zhejiang University, has met many patients who do facial filling.
Some of these patients are not very old, when they were young, their faces atrophied due to trauma and other reasons, and the other side of the face developed normally, and the children who were originally clear-eyed and had clear eyebrows became extremely unconfident and introverted and inferior because of the face on one side but did not develop.
This is "progressive hemi-facial atrophy", it is not congenital, the etiology is currently unknown. Some believe that it is related to trauma, which will slowly and progressively produce atrophy of half of the skin, subcutaneous tissue and muscles and even bones, a process that lasts 2-10 years. If not treated in time, not only will there be a lack of appearance but also a psychological impact.
In the past, the treatment of such diseases mostly used microsurgical free tissue flap transplantation for filling repair, which was particularly difficult to perform, with a high risk of surgery, bloated appearance and long knife edge scars on the thighs.
The general autologous fat grafting method is to use liposuction to suck out the belly and thigh fat during the operation, and the sucked fat is filtered and purified, and the infected part is filled with injection. However, the traditional method has more absorption (up to 30%-70%), and the absorption rate is unstable, and the surgical effect is difficult to predict.
After detailed examination and comprehensive consideration, Yi Chenggang's team decided to use the most advanced "autologous bone marrow combined with autologous fat grafting" at home and abroad for repair.
This technique is a minimally invasive operation, with small damage, fast recovery, good symmetry on both sides, and no scars in the donor area. The operation is small, and it returns to normal about 4 days after the operation, and the liposuction and injection are only small incisions, the scars are particularly small, and the chance of recurrence is very small.
The procedure of autologous fat grafting is suitable for the treatment of the face or the soft tissue of the body surface that needs to be filled, especially the treatment of soft tissue defects in the back of aging, and the cosmetic treatment of rhinoplasty, breast augmentation, buttock augmentation, and can also be used for the repair of soft tissue depression on the surface of the body.
The difficulty of treatment is that the fat absorption is high, the risk of filling operation is large, and it is also the most risky cosmetic surgery at present if the risk of vascular embolism brings disastrous consequences to the patient.
The team led by Director Yi Chenggang has been conducting autologous fat graft research since 2002, and under the funding of 4 National Natural Science Foundation of China and 5 provincial and ministerial projects, the three problems faced by the clinical application of autologous fat grafting have been studied.

2. New ways to improve the effectiveness of autologous fat grafting were explored:
Internationally, it was first confirmed that braking can improve the survival rate of autologous fat grafting, and on the basis of previous studies, a forward-looking clinical randomized controlled experiment was carried out for the first time to improve the effect of autologous fat grafting through braking. Firstly, from both animal experiments and clinical experiments, it was confirmed that autologous bone marrow can improve the retention rate of transplanted fat, in-depth mechanism research was carried out, and forward-looking clinical randomized controlled experiments were carried out; first, forward-looking clinical randomized controlled experiments were carried out for fat SVF-gel to prevent surgical incision scarring; first of all, it was confirmed through experiments that VEGF gene therapy or umbilical cord blood-derived EPC cells can improve the survival rate of transplanted fat by promoting vascularization And earlier basic and clinical research on PRF to improve the effect of autologous fat grafting.
3. Explain the mechanism of surgical complications and prevention measures:
An in-depth study of the most serious vascular embolism complications of fat grafting was carried out, and it was found that the hemodynamics of arteries played a very critical role in it, and it was proposed that the amount of fat injection at a single point was the most important factor affecting fat embolism, and the amount of fat injected at a single point was less than 0.05 ml to avoid the occurrence of severe vascular embolism. The team published 22 SCI papers in this direction, including 6 in the first international journal of plastic surgery, Plast Reconstr Surg, and 3 in the second journal Aesthet Surg J, of which 4 papers were reviewed by SCI journals, and participated in the formulation of the first group standard for medical aesthetic professions in China, "Fat Injection Transplantation Technology Standard".
