Patient: 31 years of age, male.
Main complaint: Abdominal pain began at about 4 a.m., located in the epigastric region, with cramping, severe severity, nausea and vomiting several times, without other discomfort, and continued without relief. At 8 o'clock in the morning, he visited the emergency department, the vital signs were stable, and the blood routine, blood biochemistry, troponin, amylase, etc., abdominal ultrasound and other examinations were improved, and the patient refused, asked for analgesic and antiemetic treatment, repeated persuasion was ineffective, and gastric resorcination and resorcinol targeted treatment were given.
At 11 o'clock, the patient's abdominal pain relief is not obvious, and there is chest tightness and shortness of breath, auscultation heart rate is fast, repeated persuasion to go to the blood test, while improving blood gas analysis, finger oxygen saturation examination, finger oxygen saturation is not different, transferred to the rescue room, the patient still refuses ultrasound examination. An electrocardiogram is performed immediately and the results are as follows:
At about 12:30 p.m., the cardiologist consulted, and the patient complained of pain and tingling in both lower limbs.
Examination: shortness of breath, cyanosis of the face and whole body, with the right upper limb as the mainstay, the heart sound is extremely weak, the valve noise is not obvious, the abdomen is soft, there is no tenderness and rebound pain, and the lower limbs are not swollen. ECG monitoring shows blood pressure of 89/65 mmHg, immediately perfecting bedside cardiac ultrasound.
At about 12:40, the patient lost consciousness, and the ECG monitoring showed that the heart rate gradually slowed down, became a ventricular escape rhythm, and finally died after ineffective rescue.
