In life, there are often few registration windows for the elderly, and they are told that they cannot use cash payments when they go hard, and the elderly have a short time to see a doctor, but it takes a lot of time on the road and in line. In this regard, Lin Anjie, a member of the CppcC Committee of the Beijing Municipal Committee of the China Association for the Promotion of Democracy and the former principal of the Beijing Qiushi Vocational School, said that the registration problem of the elderly in Beijing cannot be ignored, and hopes that the government will provide a more humane appointment registration platform and increase the number of registration lines for the elderly in Beijing.

"I visited several nearby communities and asked about 50 elderly people about their views on registering for medical treatment, and found that about 13-15 elderly people mentioned that they did not master the method of mobile phone registration, generally children help register; there are 23 elderly people who go to the hospital will have inconvenient situations, of which 4 elderly people have missed the number collection time because of traffic jams, going to the wrong hospital, etc.; only 9 elderly people can fully adapt to the current online appointment registration and seek medical treatment independently." Lin Anjie said.

According to the latest data from the National Health Commission, 190 million elderly people in China suffer from chronic diseases, and many of them need to go to the hospital frequently to see a doctor, change medicines, and undergo surgery. At the same time, surveys have shown that because their children are not around, more than 70% of the elderly choose to seek medical treatment alone. However, with the continuous expansion of the scale of the hospital, the increasing degree of medical informatization, and the fact that the elderly are not familiar with online registration and mobile phone payment, the problem of "difficulty in seeing a doctor" is further highlighted.

In Lin Anjie's view, the current appointment registration platform is too market-oriented and inconvenient for the elderly to use, and it is recommended that the government increase investment in this regard and purchase services. "A market-oriented platform, an advertisement is launched, and the elderly are a little chaotic." Coupled with the fact that the elderly are particularly afraid of online fraud, many apps dare not download, and the government provides a platform, and they are more at ease with their use. Lin Anjie said.

Lin Anjie also suggested that a special hotline for the elderly should be added, which should be divided into three parts: appointment registration, online consultation, and transfer to the hospital. Among them, the appointment registration is helped by professional manual or AI intelligence to register the elderly through the registration requirements and personal information provided by the elderly, and send SMS prompts after the registration is successful. In the online consultation part, through the telephone, the elderly can directly consult their own physical health, registration process and other issues, and get professional answers. Transferring the hospital part allows the elderly to communicate directly with the hospital front desk without having to go to the hospital deliberately, which not only reduces the time wasted by travel, realizes rapid docking with the hospital, and facilitates epidemic prevention and control.

In recent years, Beijing has done a lot of work in the basic old-age service, but compared with the demand, there is still a certain gap, in the view of Song Weizu, a member of the Beijing Municipal Committee of the Chinese People's Political Consultative Conference and a first-level inspector of the Beijing Municipal Committee of the China Democratic League, as a city with a relatively fast aging rate, Beijing also needs to consider the integration of hospice care into the basic old-age service system.

According to reports, hospice care is to provide physical, psychological, spiritual and other aspects of care and humanistic care services for elderly or terminally ill patients before their death, so as to control the pain and discomfort symptoms of elderly patients, improve their quality of life, and help them die comfortably, peacefully and with dignity.

According to statistics, as of the end of June 2021, there were 74 medical institutions in Beijing that carried out hospice services, providing 901 beds; 83,193 hospice nursing outpatient emergency visits; and 23,711 discharges. However, Song Weizu pointed out that "now our hospice care mainly relies on hospitals, and there are about hundreds of beds in the city, which is actually a drop in the bucket compared with the needs of tens of thousands of people." After a questionnaire sampling survey, Song Weizu learned that the proportion of elderly people who have a demand for hospice care accounts for 67.14%; hospice care as a basic old-age public service project has become an urgent need.

In terms of hospice work, Song Weizu suggested that in accordance with the "9064" pension service principle proposed in the "Beijing Municipal Regulations on Home Care Services", the "Regulations" should be supplemented and revised to incorporate palliative care into the basic public old-age service system in Beijing. Relying on community pension stations and community health centers, we will build a basic palliative care service system in our city with home care as the main body and inpatient care and day care as supplements.

Talent assurance is a top priority. Song Weizu believes that the education department should encourage and approve the opening of corresponding majors and support colleges and universities to carry out the construction of hospice care professions. Organize and support the preparation of teaching materials, and carry out flexible on-the-job training for practitioners. Form a ladder of professional and professional talent training, and provide talents for the construction of the basic service system of hospice care.

Song Weizu told the Beijing Business Daily reporter that he hopes to improve the treatment of hospice service doctors and let them serve this work with peace of mind. "Hospice care is basically based on companionship, its medication, treatment is actually already auxiliary, so there is no surgery, medication treatment and other large medical expenses, then this kind of elderly people occupy the bed, so that the hospice ward relatively speaking income is much different from other major disease treatment wards." How to solve the problem of the income treatment of medical staff and staff engaged in hospice care and solve the problem of ward turnover is also what the government needs to study. Song Weizu said.

In terms of funding sources, Song Weizu suggested that the Nursing Care Insurance Law be formulated to support insurance institutions to open corresponding insurance types, so that the cost of hospice care is jointly borne by the government, social insurance and individuals.

In addition to doing a good job in medical treatment and maintenance, the transformation of the living environment of the elderly cannot be ignored. In September 2021, the "Beijing Special Plan for Elderly Care Services (2021-2035)" was introduced, and by 2025, the city will complete the construction or transformation of 100 communities and 10,000 households with home aging facilities.

In the view of Su Hong, member of the Municipal Committee of the Chinese People's Political Consultative Conference, member of the Standing Committee of the Beijing Municipal Committee of the China Democratic National Construction Association, and deputy director of the Beijing Investment Promotion Service Center, the transformation of existing residences to adapt to aging is the primary condition for realizing home care.

"At present, the elderly population base in Beijing is large and is facing a rapid growth trend. The original home care environment is not suitable for the elderly, and the popularization of knowledge for aging transformation also needs to be improved. Su Hong said.

Deeply investigating the reasons behind it, Su Hong believes that, first of all, the Chinese people have not yet formed the concept of age-appropriate transformation, and the degree of understanding of its necessity and importance is not enough; secondly, there are problems in housing design and decoration, and some basic auxiliary facilities needed by the elderly to live or live are difficult to be presented in the transformation process; and finally, the relevant policies for aging transformation have not yet formed a synergy, and a set of perfect indoor age-appropriate transformation standards have not yet been introduced, and it is difficult to control the quality and daily maintenance of indoor reconstruction.

When talking about how to solve this problem, Su Hong suggested that the city should comprehensively publicize and promote the aging transformation of home care. According to the principle of voluntary transformation, the elderly over the age of 60 can voluntarily apply for transformation, widely publicize in the jurisdiction, focus on mapping, accurate assessment, field visits, collect feedback in the whole process of follow-up, through government subsidies, enterprise profits, family out-of-pocket fund sharing mechanism, the aging transformation work will be implemented, eliminating various concerns of the elderly.

At the same time, establish a standard system for aging transformation and a standardized management system for engineering. Improve the current aging and barrier-free design standards and management methods, make unified provisions on material selection, size, etc., and cover the mandatory standards for aging transformation to the interior of the living room. Establish an access system or supplier whitelist for suppliers and construction enterprises, establish a sound project acceptance standard and management mechanism, and strengthen the follow-up maintenance and supervision of aging renovation facilities.

In addition, establish a multi-channel funding support policy. In urban renewal and the transformation of old residential areas, we will solve the transformation of suitable aging, and include indoor and outdoor adaptation to aging renovation projects into the scope of transformation and subsidies. Provide special support to equipment suppliers and R&D providers, stimulate the vitality of market participation through relevant policies such as special funds, fund investment or enterprise tax reduction, and encourage enterprises and professionals to devote themselves to the aging engineering industry. Consideration can be given to including some age-appropriate products in the scope of social security to reduce the burden of the elderly when purchasing age-appropriate transformation services for market behavior.
