In order to restore the real living conditions of grassroots doctors, on December 20, Lilac Garden, Lilac Doctor Data Research Institute and Health Newspaper Mobile Health Research Institute jointly released the "2021 China Grassroots Doctor Insight Report" (hereinafter referred to as the report). The report spent several months surveying more than 1,000 grassroots doctors in hospitals and primary medical and health institutions in fourth-tier cities and below, and strives to reveal the truest side of the grassroots doctor group.

The report pointed out that although the grass-roots doctors surveyed generally feel that the working hours are long, the work pressure is greater, and they are more eager to improve the salary than the doctors as a whole, they also maintain the mentality of learning progress, actively carry out professional skills learning through online channels on the Internet, and the overall satisfaction with the work is not low, and the enthusiasm is not reduced.

The average working hours on weekdays exceed 9 hours

The volume of primary health services is enormous. According to the "Statistical Communiqué on the Development of China's Health and Health Undertakings in 2020" issued by the National Health Commission, under the influence of the epidemic in 2020, the number of diagnosis and treatment trips in primary medical institutions has reached 4.44 billion, the number of hospital admissions has exceeded 100 million, and grass-roots doctors are also experiencing busy lives.

The report shows that primary doctors have less disposable time, about 70% of the time per week is spent working, and the lower the job title level, the longer the working hours. They work an average of more than 9 hours on a working day. Even on rest days, primary doctors spend an average of nearly 5 hours at work.

Under the high intensity of work, grassroots doctors are very enthusiastic about learning, and the proportion of learning time in living time has reached nearly 10%. They set aside nearly 1.6 hours of study on weekdays and 2.2 hours on rest days.

In terms of social networking, WeChat is the most commonly used social platform for grassroots doctors, and Douyin ranks second. More than a third of the grassroots doctors surveyed use Douyin. Interestingly, grassroots doctors with high job titles rely more on social platforms that are mainly based on communication functions, while grassroots doctors with low job titles will use short videos, communities and other types of social platforms more.

There is a 10,000 monthly salary gap between ideal and reality

In the face of busy work, grassroots doctors are still actively engaged in professional development. The report points out that the career development momentum of grass-roots doctors is mainly based on the basic needs represented by increasing income and reducing medical risks, and the advanced needs represented by increasing knowledge, and the pursuit of high-level needs is lower than that of physicians as a whole.

Grassroots doctors surveyed said that their work troubles are mainly reflected in the problems of large workload, high requirements and low income levels. About 60 percent of doctors say they are currently working under a lot of pressure or very much. At the same time, since the beginning of this year, more than 80% of grass-roots doctors have encountered irrational medical behavior. The most common encounters are patients who are unreasonably prescribed medications, have high expectations of treatment, impose examinations, and require hospitalization. On the other hand, unreasonable requirements such as patients cutting in line and asking for certificates are issued. These irrational medical behaviors also increase the workload of doctors.

In terms of work income, the proportion of high income of grass-roots doctors is even lower, and more than 80% of grass-roots doctors hope to earn more than 10,000 yuan per month, but only 18% of doctors reach this level. According to the survey data, 79% of grass-roots doctors hope to increase their income, which is 31 percentage points higher than the overall doctor population, which shows that the need for grass-roots doctors to improve their work income is more urgent.

It is worth mentioning that more than 70% of the grass-roots doctors surveyed are satisfied with their current work. It can be seen from this that although the grass-roots doctors are generally under pressure and the objective material conditions are insufficient, their overall satisfaction with the work is not low, and they maintain a high enthusiasm.

98% of primary doctors have learned online

Learning is another important theme of grassroots doctors in addition to their work and life. Nearly 90% of primary doctors cite knowledge growth as one of the drivers of current career development, with nearly 30% of primary physicians seeing it as the most important need.

At the same time, almost all grassroots doctors have more or less problems in their daily learning, and more than half of them are facing training, learning opportunities and insufficient time. In addition, more than 30% of grassroots doctors said that they currently have low learning benefits and large economic burdens.

In terms of the content of professional learning, clinical skills and examination training are the most important daily learning content of grass-roots doctors, more than 80% of grass-roots doctors will carry out specialized clinical professional skills learning on a daily basis, and more than 30% of grass-roots doctors will learn clinical professional skills for first aid and general practice. In addition, nearly half of the primary doctors need to prepare for certification training. The proportion of primary doctors who carry out non-clinically relevant learning is relatively limited.

In the past, most of the grass-roots doctors could only obtain professional training and learning through traditional offline channels, and now, online channels have reached a high penetration rate among the grass-roots doctors. 98% of the grassroots doctors surveyed said that they had studied medical knowledge on online channels, and medical professional websites such as Lilac Garden and Medical Pulse Pass, as well as APP and medical examination training platforms were the platforms they used most often. In addition, more than half of the grassroots doctors often use auxiliary diagnosis and treatment tools such as medication assistants and pharmacopoeias, expressing a strong demand for online learning tools.
