In order to improve the level of rehabilitation of the center and allow the recovered patients to enjoy the diagnosis and treatment services of the higher-level hospital in the community hospital, the Community Health Service Center of Chedun Town and the Shanghai Fifth Rehabilitation Hospital have established a remote consultation and referral system, and established a rehabilitation medicine collaborative remote consultation center with the Shanghai First People's Hospital and the Huashan Hospital affiliated to Fudan University. On this basis, the "Full of Stars" rehabilitation team of the Community Health Service Center of Chedun Town standardizes the job responsibilities, work processes and referral processes through the management system, and provides professional rehabilitation treatment services for patients.

The community health service center of Chedun Town is equipped with 57 kinds of rehabilitation equipment on the basis of the original equipment, which can provide 60 rehabilitation services for patients. In the later stage, the rehabilitation assessment data generated in the rehabilitation treatment will also be integrated into the rehabilitation information module of the center, bringing more targeted and scientific rehabilitation treatment services to patients. "We also continue to track patients' out-of-hospital conditions, provide rehabilitation guidance, and integrate rehabilitation inside and outside the hospital by refining rehabilitation management." Qian Chunfang, director of the center, said.
