"Today, there are 20 critically ill patients in the ward. For most patients, getting into normal breathing is a difficult time. My job is to use all available means to solve all kinds of breathing problems and help them breathe normally. Yang Yongli said that the respiratory therapist is a "special soldier" in the ICU ward, and various sophisticated instruments are his weapons. Here, there are more than 20 kinds of ventilators alone, and one ventilator can change and set different modes and parameters to detect various values. Respiratory therapists must master the characteristics, applications, and maintenance of each machine.
There was a kidney failure patient in his 80s who had difficulty breathing during his hospitalization and had to clean up the sputum that had stuck to his lungs and airways as soon as possible. The effect of ordinary suction operations is not obvious, and then it is the turn of the respiratory therapist to fight. Through Yang Yongli's fine treatment, the patient's condition has improved, and recently he feels that his breathing has been unblocked. "Since the patient's lungs are not yet fully open and there is still some sputum in them, depending on the condition of breathing and coughing, I decide what instrument to use for the patient next." Yang Yongli said that a variety of pulmonary rehabilitation equipment can help patients breathe normally as soon as possible.
In the ICU ward, some patients have a particularly complex condition, and respiratory therapists must frequently discuss with doctors and nurses to update treatment plans. Yang Yongli believes that respiratory therapists are a bridge between doctors and nurses, and they work together to better treat patients.
<b>It has played a great role in the frontline of the fight against the epidemic</b>
In 2010, Yang Yongli entered the Department of Respiratory and Critical Care Medicine of the Second Municipal Hospital. In the solemn and calm ICU ward, he has trained superb skills and calm psychological quality. In 2018, under the recommendation of the department, Yang Yongli went to the Respiratory Therapy Department of Run Shaw Hospital of Zhejiang University to train as a respiratory therapist for half a year, and he was also the first professionally trained respiratory therapist in Fuyang.
In 2019, the COVID-19 pandemic broke out. Without specific drugs, the treatment of severe and critically ill patients relies heavily on life support, and respiratory therapists play a large role in the frontline.
In the treatment process, different models, different modes of ventilators, how to adapt and debug, directly determine the treatment effect, respiratory therapists must professionally adjust the operating parameters of the ventilator, so that the ventilator and the patient's lungs form a benign interaction. Taking tracheal extubation as an example, when to pull it also depends on the rigorous evaluation and timing of the respiratory therapist. "All respiratory therapy support devices are a double-edged sword, and there will be damage if you withdraw too early or too late. These details often determine the success of the treatment of critically ill patients. Yang Yongli said.
In this new crown pneumonia epidemic, respiratory therapists in Fuyang have not only participated in the treatment of Wuhan, but also made significant contributions to the frontline treatment in our city. Yang Yongli persisted until the last moment when the patient was cleared and the ward was cleaned.
Last year, the Department of Critical Care Medicine of the Second Municipal Hospital set up a respiratory treatment team. Under the leadership of the Second Municipal Hospital, other hospitals in Fuyang gradually have respiratory therapists. At present, there are more than 30 respiratory therapists in major hospitals in our city, of which 7 are in the second hospital of the city.
The outbreak has given respiratory therapists unprecedented attention, bringing this "stealthy" profession to the forefront. Yang Yongli believes that there will be more young people like him who will engage in this profession and contribute to critical care medicine with professionalism and enthusiasm.
Respiratory therapy originated from the United States, and has a history of more than 50 years of development in the United States, Canada and other countries, and has formed a certain scale of professional respiratory team. In contrast, the domestic respiratory therapy profession has just started, and the public awareness is not high. At present, only the West China Medical College of Sichuan University and the Shaw Hospital of Zhejiang University have respiratory therapy majors in China, and the team of respiratory therapists in the country is only nearly 1,000 people. Respiratory therapists play an important role not only in critical care, but also in the management of chronic diseases, in mild patients, and even in community care.
