According to reports, the Hengshui Lake Marathon was founded in 2012 and has been successfully held for 9 sessions so far. It was first held in 2012 and was awarded the Silver Medal Event of the Chinese Athletics Association, and since 2013, it has been awarded the Gold Medal Event of the Chinese Athletics Association every year. In 2019, it was rated as the World Athletics Gold Standard Event, becoming the ninth "double gold event" in China and the only one in Hebei Province.
The event was hosted by the Chinese Athletics Association, the Sports Bureau of Hebei Province, the People's Government of Hengshui Municipality, and hosted by the Sports Bureau of Hengshui City. The design scale is 20,000 people. Among them, 7,000 people in the full marathon and 3,000 people in the half marathon were determined through online registration and lottery; 10,000 people ran healthily, and the preparatory committee organized the participation of personnel from all industries and units in the city. Registration for the full and half marathon was officially launched at 15:00 on September 13, and in just two days, more than 30,000 runners participated in the pre-registration.

According to the measurement, the content of "negative oxygen ions" in Hengshui Lake is as high as 4600 per cubic centimeter, which is dozens of times that of surrounding cities, and is known as "natural oxygen bar", which is conducive to fully stimulating the physical function of athletes and effectively and quickly relieving fatigue, creating better competition results.
This year's competition identified 6 designated hospitals for medical treatment, including Hengshui People's Hospital. There are 2 first aid stations, 13 medical points (1 every 5 km, 1 at 19 km in the half-distance) and 13 medical teams (2.5 km in the whole and half-distance) at the starting and ending points and the total distance of 28 km. A total of 33 ambulances are equipped, of which 18 are responsible for the roving ambulance between the two adjacent medical points where they are located, and 2 ambulances are responsible for the whole process of roving ambulance, focusing on strengthening the rescue and treatment force of the whole 25 to 40 kilometers, so that 1 ambulance and one mobile AED emergency personnel are used every kilometer. A total of 103 ECG defibrillation devices are equipped, including 68 mobile AED. In addition, there are 58 professional pacemakers and 80 medical runners.
Hengshui City officials said that this year is the tenth time that the Hengshui Lake Marathon has been held. As the largest and most influential large-scale sports event in Hengshui City, this year has prepared six highlight activities around the theme of the tenth anniversary of the event, creating a ten-year grand festival and shaping the city festival.
