Malnutrition is a common problem in modern clinical patients, and in recent years, clinical nutrition therapy has been paid more and more attention in clinical diagnosis and treatment. Clinical nutrition therapy is important for critically ill patients, patients with certain chronic diseases, and especially for children. At present, there are more and more diseases that may cause nutritional problems in children, such as cow's milk protein allergy (CMPA), inflammatory bowel disease (IBD) and a wide variety of inborn metabolic diseases, which can also greatly affect the growth and development of children. Standardized and reasonable clinical nutrition therapy is not only a means of disease treatment, but also the key to the long-term growth and development of children.

In recent years, great progress has been made in the discipline of pediatric clinical nutrition, but there is still some room for improvement in order to allow more children to obtain standardized, timely and effective clinical nutrition treatment based on evidence-based medicine.

On April 13, the 15th Healthy China Forum on Children's Medical Nutrition, sponsored by the People's Daily Health Client and supported by Nestlé Health Science (China) Co., Ltd., was successfully held in Beijing. This forum brings together experts and scholars in the field of pediatrics at home and abroad to discuss the new progress of the international academic frontier and jointly explore the new development of child nutrition.

Since 2015, the Nestlé Health Science Nutrition Forum has been held for 9 sessions, providing a direction for the innovation and research and development of new technologies and products in the field of pediatrics in China, and greatly promoting the development of pediatric clinical nutrition in mainland China. During the forum, Professor Cai Wei, former chairman of the Parenteral and Enteral Nutrition Branch of the Chinese Medical Association and director of the Shanghai Institute of Pediatric Medicine, and Ms. Gu Xinxin, President of Nestlé Health Science Greater China, shared their views on the development, challenges and development directions of children's medical nutrition in the past ten years.

At present, the field of nutrition and health has sounded the rallying cry of "Healthy China, Nutrition First". In March 2022, the National Health Commission issued the Guidelines for the Construction and Management of Clinical Nutrition Departments (Trial) (National Health Office Medical Letter [2022]) No. 76) clearly proposes to improve the construction of clinical nutrition departments through the pilot work of the construction of clinical nutrition departments, improve the management system related to the construction of clinical nutrition departments, explore the development path of clinical nutrition disciplines in the context of high-quality development of hospitals, and continuously improve the clinical nutrition diagnosis and treatment capabilities and levels of medical institutions at all levels through the construction and guidance of demonstration units.

The "14th Five-Year Plan for National Health" issued by the General Office of the State Council clearly proposes to "implement the national nutrition plan and reasonable diet action, improve the nutrition monitoring system of residents, and strengthen nutritional intervention in key areas and key populations". Whether it's clinical medicine or nutrition, children are special groups that need attention.

In the face of the above situation, it is undoubtedly necessary for all sectors of society to do a good job in children's nutrition treatment. Nestlé Health Sciences has been holding nutrition forums for 10 years to promote the advancement of the field of pediatric clinical nutrition.

Nutrition is the material basis for ensuring the normal growth and development of children, as well as their physical and mental health. According to the 2020 Report on Nutrition and Chronic Diseases of Chinese Residents, the stunting rate of children under 6 years old in mainland China reached 7%. Ms. Gu Xinxin said, "In the past 10 years, we have witnessed the change of clinical staff from the dilemma of having no relevant products available, to a variety of special medical food options. Authoritative experts and scholars from home and abroad discussed these contents and related developments around the new progress of international academic frontiers, such as enteral nutrition support for children with diseases, diagnosis and management and prevention of CMPA, nutritional solutions for children's growth and development, and nutritional management of children with congenital metabolic diseases. Enteral nutrition support

Compared with parenteral nutrition, enteral nutrition not only better meets physiological needs in terms of nutrient absorption and utilization, but also helps maintain the integrity of intestinal mucosal structure and barrier function. Enteral nutrition is especially advantageous for infants or children who are perioperative or suffering from cerebral palsy, hematologic tumors, gastrointestinal diseases, liver diseases, severe disease, and malnutrition. The World Health Organization (WHO) advocates that enteral nutrition should be used as long as the gut is functional. It should be pointed out that insufficient nutritional intake caused by iatrogenic factors requires adequate enteral nutrition support, and tube feeding under in-hospital care is also more conducive to the recovery process, but the awareness level of patients and their families in this aspect still needs to be strengthened.

At this forum, Professor Claudio Romano, member of the Gastroenterology Committee of the European Society of Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN), introduced the nutritional management of children with impaired gastrointestinal function and the experience of how to reasonably provide enteral nutrition support for different types of children. At the same time, Dr. Jalil Benyacoub, Global Head of Medical Nutrition, Pediatric Medical Nutrition, Nestlé Health Sciences Switzerland, introduced the importance of nutrition in the management of Crohn's disease and introduced the practical application of Modulen lBD in clinical practice. In response to this, Nestlé Health Science currently has three enteral nutrition products: Oi Ershu, Xiao Bai Tai Neng and Xiao Jia Shan, which can provide enteral nutrition support for children who need nutritional support due to diseases and other reasons, and comprehensively supplement balanced nutrition.

Cow's milk protein allergy

CMPA is one of the most common food allergies in infants and young children, and it is also the most widely needed issue in pediatric nutrition management and treatment. On average, about 2~7 people in every 100 children suffer from this food-allergic disease. Of these, 90% of children have mild or moderate CMPA, which is a more accurate fit for the amino acid formula (AAF) for severe allergies. This is based on the graded diagnosis and treatment of CMPA, that is, based on the grading of the severity of the child's disease, scientific and reasonable feeding is carried out to improve the benefit of the child and improve the long-term prognosis.

However, in the face of the uneven distribution of medical resources in mainland China, it is necessary to continuously expand the volume and promote the standardized diagnosis and treatment of diseases in order to truly implement the concept of CMPA hierarchical diagnosis and treatment in clinical practice, strengthen the awareness of children's families, and establish a correct concept of feeding. At the forum, the former chairman of the ESPGHAN Gastroenterology Committee, Prof. Dr. Yvan Vandenplas shares why the advanced hydrolyzed formula (eHF) is the first choice for most children with CMPA.

In addition, breast milk is undoubtedly the best choice for infant and young child feeding, so the ultimate standard of formula is infinitely close to breast milk. At the forum, Susan Jones, head of medicine at Nestlé Health Sciences in Switzerland, said that the deep hydrolyzed formula containing purified lactose and 2 kinds of human milk oligosaccharides (HMOs) is beneficial to increase the levels of short-chain fatty acids and bifidobacteria in children, which can help the intestinal microbiota of children with CMPA and promote the development of the immune system. Nutritional solutions for children's growth and development

Affected children usually have varying degrees of insufficient nutritional intake, which can easily further lead to problems such as abnormal growth and development, and increase the burden of disease in children.
