On March 28, the Hainan Provincial Health Commission issued a notice on the "Hainan Provincial Standardized Training Management Measures for Resident Doctors".
Article 26 of the document states: explore and implement the organic connection between the standardized training of residents and the postgraduate education of the professional degree of medical master, and gradually unify the content and method of standardized training of resident physicians and the training of professional degree students of medical master's degree. Clinicians who have obtained the Certificate of Standardized Training for Residents and meet the national degree requirements can be awarded a professional degree of Master of Medicine; Graduate students who meet the requirements of standardized training and management of resident doctors, and who have been trained and qualified in accordance with the "Content and Standards of Standardized Training for Resident Doctors" formulated by the state, can obtain the "Certificate of Standardized Training for Residents".
As soon as this document with a great spirit of reform was released, it immediately caused a great sensation in the medical community, especially the medical students who are still struggling to take the postgraduate examination.

Those who have been studying for three years without a regular training certificate, aren't they as good as passing the undergraduate regular training? It's really shocking...... Anyway, as long as I have a master's degree, then why did I go to graduate school in the first place? It was a complete mistake!"

Previously, the General Office of the National Health Commission and other 4 departments issued the "Notice on Implementing the Policy of "Two Equal Treatment" for Standardized Training of Resident Doctors" (Guo Wei Ban Ke Jiao Fa [2021] No. 18), which requires that clinicians with a bachelor's degree who have passed the residency training should be treated the same as those with master's degrees in clinical medicine, stomatology, and traditional Chinese medicine in terms of title promotion.
As for how to implement this equal treatment, the graduate professional and master's examination may be a foothold.
During the two sessions last month, Academician Wang Chen, member of the Standing Committee of the National Committee of the Chinese People's Political Consultative Conference, vice president of the Chinese Academy of Engineering, president of the Chinese Academy of Medical Sciences, and president of Peking Union Medical College, proposed to the two sessions on the success and growth of medical students and young doctors.
He mainly analyzed some important problems of medical education in mainland China and their causes, and put forward six targeted suggestions and measures, among which special emphasis was placed on the establishment of an integrated physician training model of regular training and professional master's degree, professional training and professional doctorate, so as to smooth the career development path of physicians.
Indeed, at this stage, the development of medical training and medical master's education in mainland China are facing many problems.
First of all, in the five-year clinical medicine professional training program that occupies the mainstream of medical schools in mainland China, graduation internships are arranged in the final year, due to the current "unit person" system and "master's degree" examination system, medical students have to spend a lot of time looking for jobs and preparing for graduate school entrance examinations while graduating from internships, otherwise they will have no work unit or academic destination after graduation.
Secondly, following the problem of medical education in colleges and universities, after graduation, there is a simple standardized training of social residents and a master's degree in clinical medicine in the medical education stage, the former has obtained two certificates (regulatory training certificate + licensed physician certificate) after graduation, but the economic treatment is better during the study period, and the latter has obtained four certificates (regulatory training certificate + licensed physician certificate + master's degree graduation certificate + master's degree certificate) after graduation, but the economic treatment is poor during the study period, so it leads to comparison, Many contradictions and troubles in the industry have produced unstable factors, and even brewed incidents and crises.
In the end, how should we solve the above latent crises, and how should we properly handle the contradiction between the social personnel training and the professional and graduate student training?
Academician Wang Chen pointed out that a unified and standardized post-graduation medical education system should be established, a "professional person" management system should be implemented in the post-graduation education stage, and a physician training model integrating residential training and professional master's degree and professional training and professional doctorate should be implemented.
Therefore, he put forward 6 specific recommended measures:
1. Cancel the professional and master's examination for medical school graduates, and those who enter the standardized training of resident physicians, and those who take the qualification examination for medical practitioners one year later and pass (the pass rate is about 70%) will enter the professional and master's training at the same time, and after the training and study, those who pass the residency training completion examination will obtain the professional physician qualification certificate, and those who meet the requirements for awarding the professional and master's degree will be awarded the professional and master's degree certificate.
2. At the current stage, the workaround is to smooth the way for residents to apply for a master's degree with the same academic qualifications. Colleges and universities should strengthen the connection with the standardized training base of resident doctors in the region, establish a close teaching relationship, promote the sharing of regional medical education resources and complement each other's advantages, and provide convenience for residents to apply for master's degrees with the same academic ability.
Colleges and universities should support residents to participate in degree courses when they enter the training base, and residents can apply for a professional master's degree with the same academic ability as a graduate student after passing the relevant course assessment, clinical ability assessment and degree defense.
3. In the future, on the basis of the standardized training system for specialist doctors, the integration of specialist doctor training (hereinafter referred to as "specialized training") and clinical medical professional degree doctoral training (hereinafter referred to as "professional doctorate") can be carried out.
4. Improve the treatment of residential and specialized trainees. Increase financial investment, from the current 30,000 yuan per student to at least 50,000 yuan. This is the most important investment of society and is of great significance to the people.
5. Strive to promote the reform of the personnel system, and implement the "industry person" system in the stage of residential training and specialized training in the future, which can greatly stimulate the learning enthusiasm of resident and specialized physicians and improve the quality of medical education after graduation.
6. Promote the revision of the "Physician Law", change the current stipulation that medical graduates must have completed one year of clinical work before they can take the medical examination, and medical students can take the qualification examination for medical practitioners in the year of graduation, so as to avoid the legal risks of practicing medicine in the first year and smooth the career development path of physicians.
At present, Academician Wang Chen's 6-point suggestions have been included in the research of many provinces, and may be a feasible implementation plan in the future.

Let people who want to engage in basic scientific research go to graduate school, and those who want to engage in clinical work can go to regular training.
