In order to break the deadlock, Xiangxi Prefecture People's Hospital implements the department director responsibility system under the leadership of the party committee of the hospital, integrates resources such as medical treatment, teaching, scientific research and social services, introduces policies to support the construction of specialties, continuously improves hardware facilities, introduces high-endoscopic equipment, highlights the "please come in, send out" talent training, and the goal is to "enhance core competitiveness".

In July 2017, Li Xuefeng, master tutor and M.D., returned to the State People's Hospital as a high-level imported talent in Xiangxi Prefecture and served as the director of the Department of Gastroenterology, establishing "quality, safety and humanities" as the core of the development of the department's connotation, leading the team to break through the predicament and open a new situation.
Since then, the ward has begun to implement the two daily rounds of the doctor in charge, the superior doctor and the director to solve difficult and critical problems in a timely manner; the implementation of the 6-day work system of gastrointestinal endoscopy, to meet the patients within 2 days to complete the diagnosis and treatment, and at any time to solve the emergency endoscopic diagnosis and treatment; the digestive clinic 7 days a week, non-stop throughout the year, and the addition of fatty liver, cirrhosis clinic, to solve the problem of specialized treatment; every year to send 2 to 3 young doctors to the top hospitals in China to learn new technologies, and to return from school to carry out a number of provincial advanced new business.
In September 2017, the Department of Gastroenterology was officially divided into two wards, one department is mainly for the diagnosis and treatment of esophageal gastrointestinal diseases, and the second department is mainly for hepatobiliary and pancreatic diseases and inflammatory bowel diseases, with beds expanded from 50 to 104 beds; in March 2021, the reconstructed and expanded Digestive Endoscopy Center was officially put into use, providing patients with a one-stop diagnosis and treatment platform covering four levels of surgery from gastrointestinal endoscopy to ERCP (trans-endoscopic retrograde cholangiopancreatography) and ESD (endoscopic submucosal dissection).
In 2021, the gastroenterology team grasped the normalization of epidemic prevention and control on the one hand, and business development on the other, and submitted an excellent report card. In clinical work, more than 4,600 patients were discharged from the gastroenterology department, and the annual outpatient volume of the specialty was more than 47,000 person-times. The business volume of the digestive endoscopy center has grown rapidly, with more than 15,000 person-times of gastroscopy, more than 10,000 person-times of colonoscopy, more than 300 person-times of ultrasound endoscopy, and more than 3,000 times of various endoscopic treatments, which effectively solves the problem of patients' difficulty in seeing a doctor.

The diagnosis and treatment technology of difficult intestinal diseases such as inflammatory bowel disease is the commanding heights seized by every key discipline of digestive disease in China. Li Xuefeng used his doctoral research foundation and work experience in Changsha and Shenzhen major hospitals to break the situation of routine referral of inflammatory bowel disease patients in regional hospitals, and set up an inflammatory bowel disease MDT team to focus on the minority of inflammatory bowel disease, from standardizing each examination to dietary guidance, nutritional therapy, essential drugs to the use of biological agents, and implementing individualized diagnosis and treatment for each patient.
The sub-specialist team has "specialization in the art industry" and everyone has a "unique skill".

Nowadays, the Department of Gastroenterology has vigorously implemented the construction of sub-specialties, and has clearly refined the sub-specialties such as inflammatory bowel disease, gastrointestinal bleeding, early cancer of the digestive tract and ESD, biliary and pancreatic diseases and ERCP, difficult liver diseases, and digestive system tumors, and cultivated a sub-specialty team with "excellent ability and excellent business", casting a unique skill of "specialized in the art industry", making the specialty characteristics more prominent, more accurate diagnosis and treatment, and showing a hundred flowers.
Develop endoscopic technology to achieve super minimally invasive diagnosis and treatment
Digestive endoscopy is a major weapon in the development of digestive disciplines, endoscopic treatment has the advantages of super minimally invasive, fast recovery, repeatability, safety, high diagnosis rate, and reducing the economic burden of patients, which is one of the mainstream directions of the current development of digestive disciplines.
ERCP surgery is performed in patients with purulent cholangitis at 35 weeks' gestation.
In April 2021, Yang Mou, an elderly pregnant woman who was 35 weeks pregnant, was transferred to the State People's Hospital due to sudden "abdominal pain and fever" emergency department, and was diagnosed with acute purulent cholangitis of common bile duct stones through abdominal color ultrasound examination. Due to the special condition of the patient, after multidisciplinary consultation, it was agreed that the super minimally invasive ERCP surgery by the Department of Gastroenterology was the most appropriate. The Department of Gastroenterology immediately performed a fourth-level ERCP intrabile duct drainage for the patient, and a large amount of purulent bile was induced by implanting a bile duct stent during the operation, and the entire operation lasted about 6 minutes. After the operation, the patient's abdominal pain quickly relieved, the fever subsided, and he was discharged safely after 4 days.
In January 2021, 55-year-old Mr. Chen underwent colonoscopic intestinal polyps ESD surgery at the endoscopic center, completely removed polyps under endoscopy, diagnosed early rectal cancer after postoperative examination, and underwent several colonoscopic re-examinations after discharge, all of which were normal and effective.
In 2018, the Department of Gastroenterology solved the problems that have plagued more than 20 years and carried out ERCP surgery for the first time; in 2019, it tried endoscopic appendix treatment for the first time, breaking the previous surgical appendectomy method, and the pain symptoms were immediately effective; in 2020, it carried out ESD surgery independently, using the advantages of surgery to effectively reduce the pain and burden of early cancer patients. In 2020, we will independently carry out interventional diagnosis and treatment of liver cancer (hepatic artery embolization chemotherapy TACE), which effectively improves the quality of life of patients and prolongs survival. Up to now, the Department of Gastroenterology has carried out more than 1,000 cases of ERCP surgery, more than 50 cases of ESD surgery, and more than 150 cases of endoscopic appendix treatment.
This is just a microcosm of the development path of endoscopic technology in the past five years in gastroenterology. At present, the Department of Gastroenterology has officially entered the "new endoscopic era", and the level of minimally invasive diagnosis and treatment in the field of endoscopic diagnosis and treatment is at the leading level of the same level of hospitals in the border areas of the four provinces and cities.

In order to make the high-quality medical resources of digestive diseases benefit more people and expand the scope of radiation, the gastroenterology team has repeatedly carried out teaching rounds and surgical guidance in medical institutions in 8 counties, cities and townships, formed a collaborative alliance with 8 county-level hospitals such as Yongshun, Longshan and Fenghuang, and formulated a two-way referral system and extended services such as acute severe pancreatitis.
At the same time, using its own disciplinary advantages, the Xiangxi Prefecture Digestive and Digestive Endoscopy Professional Committee was established, and the annual meeting, study class and academic salon were held, aiming to comprehensively improve the diagnosis and treatment level of digestive diseases in the whole state, so that the diagnosis and treatment of digestive diseases would blossom at multiple points.

In 2021, the State People's Hospital has signed a "medical federation" alliance with the Songtao County Traditional Chinese Medicine Hospital in Guizhou Province outside the state, as well as the Yongshun County People's Hospital and the Longshan Liye Town Health Center in the prefecture, giving full play to the "bellwether" role of the three-level public hospitals, and guiding medical institutions at different levels to establish a division of labor and cooperation relationship with clear goals and clear rights and responsibilities.
