Build diagnostic models

The team of Tongji Tenth Hospital studied as many as 114,000 samples, and based on 72 characteristic values of intestinal flora such as species, function, diversity and specific strains, summarized the intestinal flora changes of 18 patients with chronic diseases such as obesity, Parkinson's disease, autism, diabetes, colorectal cancer, ankylosing spondylitis, etc., and established a diagnostic model of microbiota change characteristics, which is expected to become a standard for early diagnosis and early screening in the first line.

What's even more amazing is that patients often quietly change their gut flora before symptoms of a certain disease appear. For example, autistic children generally become ill at the age of several years, but from the results of the back-tracking results of the team of the Ten Hospitals, in fact, autistic children have problems with the intestinal flora since birth, and the intestinal flora can be used for early diagnosis and screening.

In addition, many people know that Helicobacter pylori can cause stomach cancer, and human papillomavirus (HPV) may cause cervical cancer. Scientists have found that colorectal cancer, which has risen rapidly in recent years, is also associated with a known pathogen, Clostridium nucleated.

However, the research team of the Tenth Academy found that Nucleated Clostridium in the occurrence and development of colorectal cancer and its treatment is not exactly a "villain", but a "double-edged sword". Although this bacterium promotes tumor growth through mechanisms such as immune escape, it also benefits patients from immunotherapy. If the patient's tumor tissue and stool have a higher abundance of Nucleus nucleus, the patient's immunotherapy is more effective. If the intestinal flora is "sick", how should it be treated? Transplanting the intestinal flora in healthy human feces to patients can treat a variety of diseases, which is the intestinal microbiota transplantation in modern medicine.

The formal application of intestinal microbiota transplantation started late in China. From the breakthrough of intestinal microecological detection and analysis technology to the exploration of clinical intervention of microbiota transplantation, the Tenth Hospital has long been committed to this, establishing a new theory of abnormal intestinal microbiota pathogenesis and a key technical system for clinical treatment, and obtained the approval of new technology access and management norms of the Shanghai Municipal Health Commission in 2020. The project "Creation of Classification Model of Intestinal Microbiota Diseases and Clinical Application of Microbial TransplantAtion Treatment" led by the Tenth Hospital won the first prize of the 2020 Shanghai Science and Technology Progress Award.

In this cutting-edge scientific and technological field, the ten hospitals are not satisfied with clinical research and basic research, but have embarked on the road of transformation of scientific and technological achievements and industry-university-research-medical cooperation. In July 2020, the Clinical Medicine Science and Technology Innovation Park of Tongji Tenth Hospital was completed and opened, which is the first scientific research and clinical achievement transformation platform for the construction of a public hospital in Shanghai.

The Shanghai Human Intestinal Microbiota Function Development Engineering Technology Research Center in the park was inaugurated in September 2021. The center is jointly established by Tongji Tenth Hospital, Shanghai Medicine Xinyi and BGI Gene, with the goal of establishing the first "Chinese intestinal mode strain bank" in China, and has built a 1,000 square meter GMP standard enterobacter transplant laboratory, realizing the standardized production of lyophilized powder capsules of bacteria liquid and live bacteria.

Here, the feces of the healthy donor undergo intelligent system separation, purification and other treatment, undergo strict quality control testing, and finally develop into a branch bacterium liquid and a lyophilized powder capsule of live bacteria. The bacterial fluid will be injected into the intestines of the patient through the nasal and intestinal tubes, and the patient may also take live bacteria lyophilized powder capsules. The annual production scale of the center is 150,000 standardized bacterial liquid and 300,000 live bacteria freeze-dried powder capsules, which can be used by hospitals across the country.
