Xiangya Hospital completed 1,000 cases of da Vinci robotic surgery combined with day surgery to achieve 24-hour discharge

At the opening ceremony of the 13th Xiangya Minimally Invasive Surgery Forum and the 5th Xiangya Thoracic Surgery Robot Surgery Workshop held on January 15, Xiangya Hospital was awarded the "Da Vinci Surgical Robot China Thoracic Surgery Clinical Surgery Teaching Demonstration Center".

Red Net Moment January 15 news (reporter Liu Dan intern Du Wenxin correspondent Luo Wen Wang Quanqi Zhou Yanwu) 51-year-old Ms. Li came to the Department of Chest Surgery of Xiangya Hospital of Central South University for follow-up visits and found enlarged right upper lung nodules and right lower lung nodules. Due to the proximity of the right lower lung nodule to the duanmen, conventional thoracoscopic surgery requires right lower lobe resection and right upper lung nodule removal, and Ms. Li has previously removed the left upper lobe, and conventional surgery will face serious loss of lung function.

Professor Zhang Chunfang, Director of Thoracic Surgery department of Xiangya Hospital of Central South University, used the fourth-generation Da Vinci robotic surgical system to completely remove the upper right and lower right lung nodules at the expense of 3 lung segments, and successfully retained 3.5 more lung segments (a total of 10 lung segments in the right lung of the person), laying a solid foundation for Ms. Li to maintain good breathing in her life and work after surgery. She also became the 1,000th patient to successfully undergo the fourth generation of Da Vinci robotic surgery at Xiangya Hospital.

"Protecting more lung tissue is both a requirement for patients and a higher goal for doctors. If lobectomy is performed with traditional surgery, the left upper lung needs to be removed, and the patient's normal lung segment is removed, which is a pity. Through the fourth generation of da Vinci robotic surgery, we can carry out precise positioning, we can only cut off the diseased tissue, and preserve healthy lung tissue for patients", professor Zhang Chunfang explained at the 13th Xiangya Minimally Invasive Surgery Forum and the 5th Xiangya Thoracic Surgery Robot Surgery Workshop on January 15.

In 2020, Professor Zhang Chunfang's team began to routinely carry out robotic thoracic surgery, using a surgical system of the fourth generation of Da Vinci robot system developed in the United States. The fourth generation of da Vinci surgical robot is equipped with an endoscopic system, which provides 3D high-definition images with a magnification of 5-10 times compared with traditional thoracoscopic surgery, which is conducive to the chief surgeon to judge the spatial distance. This is especially important for the delicate operation of organs, blood vessels and nerves; the wrist joints of the robotic instrument can do 540 degrees of free rotation, and the operation in a small space is a "piece of cake"; it can automatically eliminate the hand trembling of the main knife doctor, making the operation more precise and stable, the patient bleeds less, and the body recovers faster.

Through the combination of robotic surgery and the advantages of day surgery, Ms. Li was successfully discharged from the hospital within 24 hours after surgery. The reporter learned that with the help of the fourth generation of da Vinci robotic surgical system, the thoracic research of Xiangya Hospital of Central South University issued an innovative surgical method of "micro-trauma" for lung nodules across multiple lung segments. This makes it possible to combine the advantages of robotic surgery with day surgery through "surgery on the day of hospitalization, discharge within 24 hours after a short period of observation and rehabilitation after surgery", which brings high efficiency, low cost, reduced nosocomial infection, and accelerated patient recovery.

The robotic thoracic surgery day surgery of lung nodules and mediastinal tumors carried out by the team has achieved good results while also creating a precedent in related fields around the world. At the same time, the team also led the development of the National Consensus on Day Surgery of Robotic Thoracic Surgery, which provides guidelines for the development of related fields.

Professor Zhang Chunfang said that in the next stage, the team will continue to optimize and innovate robotic surgery. "It is necessary to innovate surgical methods to maximize the preservation of patients' lung function, minimize damage to the lungs, bronchi and blood vessels, and conduct more clinical research to improve the efficacy of surgery and minimize complications, while making surgery simple, shortening the operation time, and promoting the daytime surgery model deeper and farther at home and abroad."
