3. How to rescue paraquat?

Paraquat poisoning has no specific effect and requires early and comprehensive management.

First: it is to immediately prevent the toxin from continuing to absorb, if it is drunk, it must be immediately induced, thorough gastric lavage as soon as possible, at home you can first cut the throat to induce vomiting, come to the emergency room to quickly wash the stomach. Then drainage, diuresis, all to speed up the elimination of toxins.

Second: serious immediately live in the ICU, while washing the stomach, as soon as possible blood perfusion, that is, the two can be carried out at the same time, do not wait for the gastric lavage to come back to perfusion, you can start at the same time. The so-called blood perfusion, the principle is very simple, is to inject, the patient's blood out, in vitro perfusion (a machine) after washing, and then the blood back into the body, the treatment process is a bit like hemodialysis, but the use of machine filter is not the same, the purpose is to wash out the toxins.

Third: use of glucocorticoids and immunosuppressants as early as possible. Because paraquat poisoning triggers intense inflammation that targets all organs, hormones and immune-suppressing drugs are critical.

There is a special noteworthy that when we rescue patients with respiratory failure, we usually give oxygen, but when paraquat poisoning patients have chest tightness and hypoxia symptoms, they do not routinely give oxygen (when I first saw this description, I was also blinded), generally do not advocate oxygen, why? Why is hypoxia not given oxygen therapy? Because paraquat poisoning can form a large number of oxygen radicals in the lungs, existing studies are worried that oxygen inhalation will aggravate paraquat toxicity, add poison to poison, and exacerbate lung damage, so it is recommended that paraquat poisoning be unconventional oxygen therapy.

No oxygen therapy does not mean that oxygen cannot be inhaled. If the patient is about to die of lack of oxygen, and if he does not inhale oxygen and die immediately, inhaling oxygen will increase the risk of death, how does the doctor choose? Needless to say, it must be oxygen. Therefore, the current guidelines recommend that oxygen should be considered unless the patient's partial pressure of blood oxygen is < 40 mmHg (normal 90-100 mmHg, < 60 is considered for respiratory failure), and it is to be inhaled at low concentrations. Very severe patients may require endotracheal intubation with ventilator-assisted ventilation, but studies have shown that ventilators do not increase survival, but only dying struggles.

But if patients have the opportunity to do lung transplantation, then ventilators are still helpful, and even use ECMO (artificial lungs), because these respiratory support techniques can prolong survival, to put it in layman's terms, it is to continue life and wait for lung transplantation.

4. Can a lung transplant be done once and for all?

Nothing more. Paraquat has been banned from sale.

It's also a bit sad to say.

Since 2016, the Ministry of Agriculture has announced the cessation of the domestic sale and use of paraquat water agents. On 29 June 2017, the Ministry of Agriculture issued a response to Recommendation No. 3326 of the Fifth Session of the Twelfth National People's Congress, which mentioned that the registration of paraquat products would no longer be approved.

On September 25, 2020, the General Office of the Ministry of Agriculture and Rural Affairs issued the Notice on Effectively Strengthening the Special Rectification of Paraquat.
