After admission, after the relevant examinations and tests are improved, the diagnosis is clarified, and the contraindications to surgery are ruled out, and the next day, the "in situ heart transplantation" is performed under the extracorporeal circulation of intubation general anesthesia. The surgery went smoothly and the heart after the transplant resumed.

After surgery, he was transferred to the cardiac surgery intensive care unit for further monitoring and treatment. Due to the patient's long-term cardiac insufficiency before surgery, pulmonary hypertension and pulmonary vascular resistance are high. Patients are very likely to have blood from the right heart system not reaching the left heart, hypotension, hypoxemia, right heart failure, systemic edema, low urine output and even the need for hemodialysis.

After the patient returned to the cardiac surgery ICU, Liu Daxing, director of the Cardiovascular Surgery Department of the Affiliated Hospital of Zunyi Medical University, instructed: immediately use NO inhalation, maxitentan, and sripag to help reduce the pressure of the pulmonary artery, accurately control the amount of fluid in and out, strengthen diuresis and other measures to improve the right heart function, floating catheters to continuously monitor the pressure of the pulmonary artery, and at the same time give the patient a series of measures such as deep sedation, avoid high fever, and avoid internal environmental disorders.

On the third day after the operation, Ms. Zou was conscious, her muscle strength recovered, her respiratory circulation was stable, her internal environment was satisfied, her urine output was satisfactory, and the ultrasound of the bedside heart prompted that the patient's left and right heart functions were satisfactory, and she successfully removed the ventilator and pulled out the endotracheal intubation.

On the fourth day after the operation, Ms. Zou was able to go to the ground to move, and her spirit and diet were the same as normal people.

Of course, because Ms. Zou needs to use immunosuppressants for transplantation, but the use of immunosuppressants will make the infection high, the digestive tract easy to bleed, and the liver and kidney function be impaired, so the anti-rejection response and anti-infection must be taken into account at the same time. Every operation and disposal is strictly sterile, and the adjustment of each drug is careful and closely monitored.

In the second half month after the operation, Ms. Zou was satisfied with the improvement of heart function, good urine output, no infection, no post-transplant rejection reaction, no gastrointestinal bleeding, and satisfied liver and kidney function. He was transferred out of the intensive care unit in the second half month of surgery.

At present, Ms. Zou has no fatigue, shortness of breath, and no lower extremity edema, and can move freely in the ward. The quality of life has been significantly improved, and the blood and cardiac ultrasound indicators are relatively satisfactory. Ms. Zou and her family smiled with satisfaction.
