In the early morning of May 10, an emergency call from the stroke green channel was sent to the neurosurgery department: in the hospital CT room, a patient with cerebral hemorrhage was unconscious and needed immediate consultation with a neurosurgeon.
The patient is Ms. Zhang, 53 years old. Her family lived in Wuchang District, Wuhan City, at 6:30 a.m. that morning, the family found that her side of the body could not move, could not speak, consciousness began to blur, and vomited, so they rushed her to the Third People's Hospital of Hubei Province.
"Cranial CT shows massive bleeding in the right side of the brain, and it is judged that the right basal ganglion intracerebral hemorrhage has broken into the ventricles, bilateral mydriasis, weak spontaneous breathing, and blood pressure as high as 219/120 mmhg." After Ms. Zhang was sent to the hospital, the emergency department's diagnosis showed that her condition was very critical and the situation was dangerous.
Professor Li Jun, director of neurosurgery, believes that Ms. Zhang must undergo surgery immediately, otherwise she may die at any time, but the risk of this operation is also extremely high, and even if the operation is successful, the probability of survival in the end is difficult to say what it is. After urgent communication with Ms. Zhang's family, the family decided to support the doctor's decision to perform surgery and fight for even the slightest chance of survival for her.
Thrilling, 4 hours of surgery to save a life
For patients who have bilateral mydriasis, the conventional surgical method is to directly perform surgery to remove the hematoma, and part of the skull needs to be removed to reduce the pressure, but this process usually takes 1-2 hours, in ms. Zhang's case, it is likely that even the preoperative preparation time can not be supported. In order to shorten the herniation time as much as possible and strive for greater protection of brain function, Li Jun decided decisively to directly drill holes at the bedside to reduce bleeding and reduce the pressure in the brain, and then to do craniotomy in the operating room.
Extremely quickly, the neurosurgery team of the hospital split into two ways. The first group of centaurs quickly drilled the skull at the side of the nicc bed to induce bleeding and reduce intracranial pressure, and placed an intracranial pressure monitoring probe to monitor intracranial pressure in real time, protecting brain function as much as possible and gaining valuable time for subsequent surgery. After the drainage of part of the hematoma, Ms. Zhang's intracranial pressure dropped from 60mmhg to 12mmhg, blood pressure dropped from 219/120 mmhg to 120/60mmhg, and the dilated bilateral pupils returned to normal size. At the same time, the second group of personnel prepared for craniotomy in the operating room.
Subsequently, Ms. Zhang was admitted to the operating room and underwent right frontotemporal top cerebral hemorrhage removal and bone flap decompression. After a thrilling 4 hours, the operation was declared a success.
In a coma for two months, no one gave up
Ms. Zhang left the operating room safely and was admitted to nicu (neurosurgery intensive care unit), but the trials and difficulties faced by the treatment were not over. After the operation, she remained in a deep coma, breathing weakly on her own and unable to get out of the ventilator. Long-term bed rest also caused her to have a series of complications such as deep vein thrombosis of the lower limbs, repurposed atrophy of the limbs, and joint contractures.

The treatment process is the efficient operation of a complete system. The medical staff of the neurosurgery department guarded the bedside day and night, closely monitoring the changes in the condition; in order to prevent the occurrence of serious pneumonia with a ventilator for a long time, nicu doctors, nurses, and rehabilitation technicians worked together to promote sputum drainage for Ms. Zhang; in order to avoid Zhang Dajie's joint contracture deformity, the medical staff intermittently moved the limbs of the elder sister every day; in order to improve the patient's own immunity and resist the complications of infection, the department nutritionist formulated a nutritional diet plan for the patient and adjusted the recipe at any time according to the situation; in order to let the elder sister wake up, Doctors remind family members to record various sounds to stimulate patients...
A month later, Ms. Zhang is still in a coma, which makes the family feel that there is little hope. They cried and asked, "Can't you wake up?" Can she still wake up? ”

After another month, a miracle happened! "When I approached my mother's hospital bed that day, I called out to her. Suddenly, I saw her eyelids move, and I knew that she was really alive! Sister Zhang's daughter said that at that time, she was so excited that her whole body was trembling.
On July 20th, after two months of suffering, after systematic awakening and rehabilitation treatment, Ms. Zhang woke up!
On the 22nd, she was able to express and speak. Next, through exercise, she will restore her speech and self-eating ability step by step.
