The patient is Ms. Zhang, 53 years old. After urgent communication with Ms. Zhang's family, the family decided to support the doctor's decision to perform surgery and fight for even the slightest chance of survival for her.

Thrilling, 4 hours of surgery to save a life

For patients who have bilateral mydriasis, the conventional surgical method is to directly perform surgery to remove the hematoma, and part of the skull needs to be removed to reduce the pressure, but this process usually takes 1-2 hours, in ms. Zhang's case, it is likely that even the preoperative preparation time can not be supported. In order to shorten the herniation time as much as possible and strive for greater protection of brain function, Li Jun decided decisively to directly drill holes at the bedside to reduce bleeding and reduce the pressure in the brain, and then to do craniotomy in the operating room.

Extremely quickly, the neurosurgery team of the hospital split into two ways. The first group of centaurs quickly drilled the skull at the side of the nicc bed to induce bleeding and reduce intracranial pressure, and placed an intracranial pressure monitoring probe to monitor intracranial pressure in real time, protecting brain function as much as possible and gaining valuable time for subsequent surgery. After the drainage of part of the hematoma, Ms. Zhang's intracranial pressure dropped from 60mmhg to 12mmhg, blood pressure dropped from 219/120 mmhg to 120/60mmhg, and the dilated bilateral pupils returned to normal size. Subsequently, Ms. Zhang was admitted to the operating room and underwent right frontotemporal top cerebral hemorrhage removal and bone flap decompression. After a thrilling 4 hours, the operation was declared a success.

In a coma for two months, no one gave up

Ms. Zhang left the operating room safely and was admitted to nicu (neurosurgery intensive care unit), but the trials and difficulties faced by the treatment were not over. After the operation, she remained in a deep coma, breathing weakly on her own and unable to get out of the ventilator. Long-term bed rest also caused her to have a series of complications such as deep vein thrombosis of the lower limbs, repurposed atrophy of the limbs, and joint contractures.
