In order to strictly abide by the bottom line of mine safety production, the State Mine Safety Supervision Bureau recently carried out comprehensive supervision of mine safety production in key areas in China. In a random spot check of the former banner of Urat in Bayannaoer City, Inner Mongolia, the inspection team found that some safety facilities in some underground mines were useless, and there were many major hidden dangers for a long time.
Self-rescue devices are useless Underground mines are full of hidden dangers
Located in the qianqi of Wulat, Inner Mongolia Yuanmeng Mining has three mining areas and three tailings reservoirs. In the northern mining area of Baledegou, an iron ore mine with an annual production scale of 400,000 tons, the inspection team found that there was a clear discrepancy between the two disaster avoidance road maps at the entrance of the mine. The plan of the middle section of the 1550 pingtong on the right shows that the roadway basically ends at the pedestrian ventilation shaft on the left, while in the vertical profile of the whole mine area of the same proportion, this roadway continues to extend for several hundred meters after passing through the pedestrian ventilation shaft. The disaster avoidance map indicating the escape route could not be understood by the mine managers.
The inspection team entered the underground to inspect and found that multiple mining holes on both sides of the roadway were blocked, and the actual survey map surveyed a week ago was not marked. The pedestrian ventilation patio, which serves as a safety exit for the underground, also does not match the actual location.
According to the requirements, the underground mine must be equipped with the mine self-rescue device. The inspection team not only did not see the markings on the drawings, but when they tried to find them, they found that these life-saving devices had become decorations.
There is no water in the water supply system, and the self-help device that provides air next to it is stuck by a pipe and cannot be opened at all.
Guo Yan, deputy director of the Non-Coal Mine Department of the State Mine Safety Supervision Bureau: This (pin) can't be moved, this is moving, this place can't be opened, blocked, blocked behind the pipe. It is said that there is that pressure air mask here, if there is a case that you can't run out, you can pick up the pressure air mask to inhale, and it is better to install this place than not to install it.
To ensure safety, downhole personnel must carry dangerous gas detectors. The inspection team saw in another Hailiusi mining area of Yuanmeng Mining that the entrance to the mine clearly indicated that there were four toxic and harmful gases under the well, and the detector carried by the staff could only detect three.
During the inspection process, the mine employees either answered the questions or did not know what they asked. Many managers of Yuanmeng Mining could not understand the drawings and could not speak clearly. The inspection team asked to check the academic background and professional license of the technical personnel, but the enterprise never provided it.
Guo Yan, deputy director of the Non-Coal Mine Department of the State Mine Safety Supervision Bureau: Whether the technicians have a professional background, they have not seen (license) until now. Originally, mines were high-risk industries, and we didn't even have a few people who understood.
Intermediaries cheat And local regulatory reviews are inadequate
During the downhole inspection, the inspection team found that there were 5 and 4 major safety hazards in the two mining areas of Yuanmeng Mining. According to the regulations, mining enterprises should ask professional institutions to conduct safety design and acceptance evaluation, and report to the regulatory authorities for review. How does such a hidden mining enterprise design and pass the safety acceptance? How is it regulated locally? The inspection team conducted in-depth verification.
Hebei Xinye Engineering Technology Co., Ltd. for Yuanmeng Mining issued a mining area safety facility design report, the design content seems to be complete, but the inspection team experts carefully checked and found that a large number of key information is missing, avoiding the heavy and light problems are serious.
The inspection found that the design unit arbitrarily changed the original design to meet the needs of the owner, not the enterprise organized production according to the safety design, but the design was changed according to the actual production situation, so as to achieve the purpose of covering up the original illegal production.
The safety design is irresponsible, and the safety evaluation report is also seriously injected with water. The experts of the inspection team found that the safety evaluation acceptance report issued by Jilin Baohua Safety Evaluation Co., Ltd. for the tailings reservoir of Yuanmeng Mining was seriously inconsistent with the actual situation.
Li Quanming, an expert of the National Mine Safety Supervision Group: The recent safety status evaluation report hardly corresponds to the actual tailings reservoir. The entire evaluation report is thirty or forty pages long, with five pages in the main body and five pages still repeating the previous content. I just don't see a point that is closely integrated with the status quo.
The many hidden dangers found by the inspection team underground are all qualified in the underground mine evaluation report issued by Jilin Baohua. The report also made a vague evaluation of the mine's lifting system, which was clearly fraudulent.
Li Fuwei, an expert of the National Mine Safety Supervision Group: I can't see the problems found on the site at all in the safety acceptance evaluation report, and some reports also evaluate the lifting system, but the mine does not have a lifting system.
The inspection found that there was a serious shortage of professionals in the local regulatory departments, and it was difficult to strictly control the design and evaluation reports of the enterprises.
After the three-year action of special rectification of safe production is launched in 2020, enterprises are required to carry out a large-scale investigation of hidden dangers, self-examination and self-correction and monthly reporting. The inspection found that the self-inspection and self-correction table reported by Yuanmeng Mining not only had zero major safety hazards for a long time, but also obviously flowed into the form.
Wu Ren'ou, first-level director of the Non-Coal Mine Department of the State Mine Safety Supervision Bureau: Every month's table (content) is the same. Even the number of general hidden dangers is the same. The number of tailings reservoirs is 2 at a time, and you have reported for three consecutive months, and only the dates are different for three months.
For the self-inspection and reporting of enterprises, the local regulatory authorities have not carried out strict verification. The local emergency management department's 2020 inspection found that there were 32 hidden dangers in Yuanmeng Mining, including major hidden dangers, but only required to be ordered to rectify within a time limit, and did not punish.
Guo Yan, deputy director of the Non-Coal Mine Department of the State Mine Safety Supervision Bureau: At present, we are going to inspect, and these problems are still in those places, indicating that some problems have been checked out and some problems have not been found. Some problems were checked out, and there was no staring to rectify it in place.
Major hidden dangers mine suspension and rectification The intermediary agencies involved were investigated
According to the problems found by the inspection team, the local regulatory authorities required Yuanmeng Mining to stop production and rectify the underground mine in accordance with the law, and re-carry out safety design and acceptance evaluation. The inspection team will also issue a list of intermediary institutions that have made false reports, notify the local regulatory departments, and investigate and deal with them in accordance with the law.
The setting of mining rights is not scientific Supervision needs to form a joint force
In addition to Yuanmeng Mining, the inspection team divided into multiple roads and carried out random inspections of a number of mining enterprises in the Inner Mongolia Autonomous Region. The inspection found that the setting of some mine rights is unscientific, which also lays hidden dangers for the later stage of safe production, and the safety production of mines urgently needs the joint efforts of multiple departments.
At Inner Mongolia Huatuo Mining Co., Ltd., the inspection team walked for nearly an hour to go deep into the Dianlisi Taijing iron mine, which was being built. In the darkness, workers were working with the help of faint miner's lamps. Infrastructure mines have not formed ventilation, power supply, drainage systems, so that workers down the well, there are major risks and hidden dangers.
Dianlisi iron ore is a 90,000-ton small iron ore transformation acquired by Huatuo Mining in 2019. Underground inspection found that the original roadway was full of blocked mine holes, and in the past there were illegal acts of mining while building and mining beyond the scope, and the waste of mineral resources was serious.
Pei Wentian, deputy director of the State Mine Safety Supervision Bureau: Those are all old mining (left by enterprises) in the past, and they all went in the past to dig up when they saw the ore body. Basically, 15 meters are mined, and then cannot be mined, and one-third of the resources cannot be used, and all are wasted. It has formalities, but it does all illegal mining.
The existing mining license production scale of the Dianlisi Tai Iron Mine is 200,000 tons per year, and the inspection team checked the data and found that the west, southwest, east and southeast sides of the mine have mining rights. Multiple mining licenses have been issued locally in the same mining belt, belonging to four mining enterprises. Each mining license is small in scale and limited in scope, resulting in a congenital lack of security design for enterprises.
The inspection found that the local lack of long-term planning for mine development, in recent years, the new batch of mining fields is still divided into small areas, "large mines are small open" and "one mine is more open", and enterprises compete for resources for each other in pursuit of benefits, resulting in violations of laws and regulations such as over-layer cross-border mining.
Pei Wentian, deputy director of the State Mine Safety Supervision Bureau: Originally, if I had said that I had understood the middle section of three or five, I would have designed it once, and I would have spent a one-time money (design fee) to build a mine, and I would have been able to solve the problem. Now at every level (middle stage), I spend money (design), build a mine, the first money is more, the second efficiency is low, the company must lose money. What to do if the company loses money, it will definitely not do it according to the design. Your mining rights settings cannot meet the needs of normal design and production of enterprises.
In one week, the inspection team conducted spot checks on 7 mines and 6 tailings reservoirs in Inner Mongolia, and found a total of 93 hidden dangers, including 14 major hidden dangers. At the same time, it was found that there were serious shortages of professional supervisors, weak supervision and law enforcement, and unscientific setting of mining rights in the early stage, and feedback to the autonomous regional government was given supervision opinions.
Guo Yan, deputy director of the Department of Non-Coal Mines of the State Mine Safety Supervision Bureau: Overall, we feel that the situation of mine safety in Inner Mongolia is still not optimistic, and small mines account for about 75%. It is recommended to promote the relevant departments to perform their duties and responsibilities, improve the minimum mining scale and minimum service life standards of mines, and solve the problems of "one mine and more opening" and "large mines and small openings" from the source.
The inspection team stressed that the fourth quarter is the traditional peak production season of mining enterprises, and it has always been a high incidence period for accidents. This year, under the "dual drive" of "guaranteeing supply" and interests, the impulse of illegal production and construction has rebounded, and the risk of accidents will be further intensified. Localities should strengthen bottom-line thinking and make overall plans for energy supply and safe production。
Pei Wentian, deputy director of the State Mine Safety Supervision Bureau: I hope that several of our departments will unite to increase the intensity of law enforcement during this period of "guaranteeing supply", and under the circumstance that the strength of each department is not strong and there is a shortage of personnel, I think that under special circumstances, we must give play to the advantages of our departments.

As of October 25, in response to the problems found by the inspection, the supervision and law enforcement department of Inner Mongolia has ordered 4 mining enterprises to stop production and construction, and all those with major hidden dangers have stopped production and construction and rectification, ordered 9 to be rectified within a time limit, and implemented an administrative penalty of 1.343 million yuan.
