Basic information of the accident unit
1. Wuxi Tiantianrun Textile Technology Co., Ltd. Cui Jiangang is the chairman and general manager, Cui Jianfeng is the person in charge of production, Cui Fuliang is responsible for logistics management, and Wu Guangqian is the safety officer. At the time of the fire, 10 people in the company's twisting workshop, 22 people in the Icelandic wool workshop, 8 people in the crochet workshop, 14 people in the velvet workshop and 3 mechanics all escaped.
2. Huishan District Qianzhou Aofeng Yarn Factory. The factory contracted the rewinding process business of Tiantianrun Company, and produced in the fourth floor workshop of Tiantianrun Company's warehouse, with a total of 10 rewinding machines. At the time of the fire, all seven employees of the plant were killed.
How the accident happened
At 06:52 on November 20, Xu Jianmin and Dai Xiaoming, the car blocking workers of the flower twisting workshop of the large workshop, arrived at the workshop and entered the No. 5 flower twisting machine station to start work.
At 18:25:29, there was a flash in the area near the north window on the west side of the No. 5 flower twisting machine in the flower twisting workshop.
At 18:25:40, the fire was visible in the north area of the No. 5 flower twister, and then the fire in this area intensified, and the fire developed rapidly from low to high, and the smoke spread.
Xu Jianmin and Dai Xiaoming ran to the passage to extract the fire extinguisher at 18:25:47 and 18:26:01 respectively to extract the fire extinguisher and enter the west side of the No. 5 flower twisting machine to extinguish the fire.
At 18:29, the squad leader Chen Derong reported to Qian Koufang (director of the flower twist workshop) by phone, and then Qian Koufang reported to Cui Jiangang by phone.
Cui Jiangang rushed to the scene from his home, used a fire extinguisher to extinguish the fire, and after seeing that the fire could not be extinguished, he asked employees to evacuate.
At 18:34, Cui Jiangang dialed "119" to call the police.
Cause and nature of the accident
The accident investigation team determined that the cause of the fire accident was: during the production of Tiantianrun Company, the friction of the components in the operation of the drafting mechanism in the west and north of the No. 5 flower twisting machine in the large workshop flower twisting workshop generated high temperatures, igniting combustibles such as cotton fibers, wool wool and acrylic fibers; The fire spread rapidly to the warehouse through the two-story illegal building connected to the north side, and the high-temperature, toxic and harmful smoke generated by the fire filled the building and the evacuation stairwell. Tiantianrun Company failed to organize the evacuation of employees in the four-story workshop in the warehouse in a timely manner, and the safety exit of the outdoor evacuation staircase was locked, resulting in the death of 7 employees.
The investigation determined that the "11.20" large fire accident of Wuxi Tiantianrun Textile Technology Co., Ltd. was a production safety liability accident.

The main problems that exist with regard to the responsible units
(1) The accident enterprise
The main responsibility for safety production and fire safety responsibility of Tiantianrun Company has not been implemented, the fire safety responsibility system for all employees has not been established, the safety risk identification and control of the flower twist workshop is not in place, the fire protection facilities in the factory area have not been built according to the design review opinions, and a number of illegal buildings have been built to occupy the fire spacing, and the safety exit of the outdoor evacuation staircase on the fourth floor of the warehouse has been locked.
(2) Relevant departments
1. Huishan Branch of the former Wuxi Municipal Planning Bureau
In July 2005, the Huishan Branch of the former Wuxi Municipal Planning Bureau approved the design width of the velvet workshop from 30 meters to 40 meters and the 4-storey structure of the warehouse to 3 floors when handling the planning permission for the construction project of the plant (velvet workshop) and the warehouse.
2. Former Huishan District Construction Bureau
In July 2005, the former Wuxi Huishan District Construction Bureau issued the construction permit of Wuxi Hongyun Textile Factory plant (velvet workshop) and warehouse construction project, with 3 floors of warehouse and 2,304 square meters of construction area. After the construction permit was issued, the quality supervisor did not supervise the construction process.
3. Huishan District Fire and Rescue Brigade
In January 2008, the former Wuxi Huishan District Public Security Fire Brigade accepted the fire protection of the warehouse, and the site has been built with 5 floors, which is inconsistent with the number of layers of planning permission and construction permit.
In April 2015, the Huishan District Brigade of the former Wuxi Public Security Fire Detachment inspected Tiantianrun Company, and in December 2022, the Huishan District Emergency Bureau and the Fire and Rescue Brigade conducted a joint inspection of Tiantianrun Company, and found that the company built illegal buildings to occupy the problem of fire spacing.
The Huishan District Fire and Rescue Brigade did not supervise and guide Qianzhou Street in performing its fire safety inspection duties, and when guiding Qianzhou Street to implement the fire supervision body, the training of grading standards was not in place, and the record list was not strictly controlled.
4. Huishan District Emergency Management Bureau
Failed to effectively supervise and urge the Qianzhou Street Emergency Bureau to implement the special rectification requirements, and did not supervise and guide the safety production work of the Qianzhou Street Emergency Bureau.
(3) Local Party committees and governments
1. Huishan District Government
Failed to conscientiously learn the lessons of the "11.21" particularly major fire accident in Anyang, Henan Province and the "11.16" fire accident in Luliang, Shanxi, and failed to combine the actual situation in Huishan District and draw inferences from one case to another. The responsibilities of fire supervision are not clear, and the relevant departments, units and the party committee and government of Qianzhou Street are not in place to perform their production safety and fire safety management duties, and the guidance and supervision of Qianzhou Street's performance of local production safety and fire safety management responsibilities are not effective.
2. Qianzhou Street
(1) Failure to conscientiously implement the responsibility of territorial supervision, insufficient attention to the professional capacity building of safety production and fire safety supervision and inspection personnel, and the safety production and fire safety post personnel equipped with the street did not perform their duties as required, and the production safety and fire safety of Tiantianrun Company were out of control and leakage.
(2) The special rectification is not solid, and in the special inspection of industrial enterprises such as metallurgy, textile, printing and dyeing, and the special joint law enforcement inspection of the "life channel" of labor-intensive enterprises, the self-inspection and rectification of hidden dangers of Tiantianrun Company have not been verified and spot checks have been organized; In the work of fire safety rectification and improvement of industrial enterprises, Tiantianrun Company was not urged to carry out self-inspection and self-correction; The implementation of safety risk identification, control and reporting of Tiantianrun Company was not inspected, and the fire hazards of Tiantianrun Company were not discovered and eliminated for a long time.
(3) Failure to effectively carry out the investigation and investigation of the occupation of fire spacing, fire passages, and illegal construction of buildings (structures) by industrial enterprises, and the illegal construction behavior and illegal construction of Tiantianrun Company were not discovered in a timely manner.
(4) In the implementation of the annual work task list of fire protection in Huishan District, the spirit of "double filing" is not mastered, the fire protection grading supervision standards are not familiar, and the nature and scale of the unit are not carefully verified, and the fire supervision body of Tiantianrun Company is incorrect.
Suggestions for dealing with relevant units and responsible persons
(1) Persons for whom compulsory measures have already been employed by judicial organs
Cui Jiangang (chairman and general manager), Wu Guangqian (safety officer), Cui Fuliang (logistics management personnel), Cui Jianfeng (production leader) of Tiantianrun Company; Gu Huifeng and Yu Qiuying, the persons in charge of Qianzhou Aofeng Yarn Factory, have been taken compulsory measures by the public security organs on suspicion of the crime of major liability accidents.
(2) Relevant responsible personnel
Clues and related materials such as problems in the performance of duties by 14 relevant responsible personnel of the local party committee and government and relevant departments found in the investigation of the accident have been handed over to the accident responsibility investigation and investigation team of the Wuxi Municipal Commission for Discipline Inspection and Supervision for disposal.
(3) Administrative punishments
Tiantianrun Company is responsible for the occurrence of the accident, and it is recommended that the relevant departments give administrative penalties in accordance with the provisions of the "Work Safety Law of the People's Republic of China" and the "Fire Protection Law of the People's Republic of China".
(4) Other handling suggestions
