Guidelines for Licensed Pharmacists in Pharmaceutical Retail Enterprises

Chapter I: General Provisions

Article 1 In order to guide and help licensed pharmacists in drug retail enterprises to carry out pharmaceutical services and ensure the safety and effectiveness of drug use by the public, this guide is formulated in accordance with the Drug Administration Law of the People's Republic of China and the professional qualification system of licensed pharmacists.

Article 2 These guidelines are applicable to licensed pharmacists in pharmaceutical retail enterprises.

Article 3 Pharmacy service refers to the behavior of licensed pharmacists of drug retail enterprises who apply professional knowledge, skills and tools in pharmacy to provide direct and responsible drug-related services to the public, with a view to improving the safety, effectiveness, economy and suitability of drug treatment.

Article 4 Licensed pharmacists should insist on providing pharmaceutical services to the public, and continue to improve the quality and level of pharmaceutical services to continuously meet the public's demand for rational drug use.

Chapter II: Basic Requirements

Article 5 When licensed pharmacists provide pharmaceutical services, they shall protect the privacy of the service recipients, and clearly indicate the content, process, and method of the service.

Article 6: Licensed pharmacists may assist in carrying out prescription review and pharmacy services through computer information systems, record the information of prescriptions and service recipients and the process of pharmaceutical services, do a good job of protecting the personal information of service recipients, and ensure that relevant records are true, accurate, complete, and traceable.

Article 7 Licensed pharmacists shall use pharmaceutical tools and software, professional reference books, and teaching aids for drug use to carry out pharmacy services, meet the needs of pharmaceutical services, and ensure the quality of pharmaceutical services.

Article 8 Licensed pharmacists shall carry out pharmaceutical services in accordance with the pharmacy service management system, operating procedures and etiquette norms.

Article 9: When licensed pharmacists carry out online pharmacy services, they shall carry out them in a real way, and shall not replace services with artificial intelligence programs;

Chapter 3 Service Content

Article 10 Pharmacy services include prescription dispensing, medication guidance, adverse drug reaction monitoring, health education, etc.

Article 11 Prescription dispensing includes prescription review, prescription dispensing, prescription review, drug delivery, and drug accounting.

Article 12 When a licensed pharmacist reviews the prescription, if there are inappropriate drug use problems such as repeated administration, overdose administration, compatibility contraindications, etc., the service user shall be informed to contact the physician of the department to confirm, and the prescription should be refused to be adjusted if it is found that it is seriously irrational or wrong.

Article 13 After dispensing the prescription, the licensed pharmacist shall check whether the name, specification, dosage form, dosage and usage of the prescription and the dispensed drug are consistent one by one, check the appearance quality and expiration date of the drug, and confirm that the drug can be delivered only after confirming that it is correct.

Article 14 When selling drugs under special management and drugs with special management requirements of the state, the licensed pharmacist shall review the prescription and register it in accordance with the relevant provisions of the state.

(1) Selling Class II psychotropic drugs in prescribed doses, and must not sell Class II psychotropic drugs to minors.

(2) When selling compound preparations containing special drugs, prescription drugs shall be sold strictly by prescription, and the quantity of non-prescription drugs sold at one time shall not exceed the requirements of relevant national regulations. If false prescriptions, prescriptions that exceed normal medical needs, and large quantities or multiple purchases of such drugs are discovered, the licensed pharmacist shall advise, stop and refuse, and report to the local department responsible for drug supervision and administration.

(3) When selling drugs containing stimulants, they shall verify the labeling of "athletes should use with caution" in the drug instructions and labels, and inform the service recipients of "athletes should use with caution".

(4) When selling toxic Chinese medicine decoction pieces, the measurement shall be accurate, and the prescribed dose shall not be exceeded, and the processed products shall be paid if the prescription is not indicated as "raw use".

(5) If the enterprise where the licensed pharmacist works provides decoction services for traditional Chinese medicine, the licensed pharmacist (Chinese medicine) is responsible for guiding and supervising to ensure that the quality of the decoction pieces used meets the requirements.

Article 15 When delivering drugs, licensed pharmacists shall explain and guide the use of drugs in accordance with the instructions and prescription usage.

Article 16: Licensed pharmacists shall sign or affix a seal to the prescription when conducting prescription review and dispensing, and the electronic signature on the electronic prescription note shall be verified by electronic real-name authentication.

Article 17 The drug dismantling service shall comply with the requirements of the Good Manufacturing Practice for Pharmaceutical Operations, and the workbench and tools for dismantling shall be kept clean and hygienic to prevent confusion and errors, and the sales record of dismantling shall be made. When selling drugs that are dismantled, patients shall be informed of the dosage and precautions of the drugs, and the original or copy of the drug instructions shall be provided.

Article 18 Licensed pharmacists shall use the professional knowledge and tools they have mastered to answer the public's medication consultation, provide individualized medication guidance, improve patients' medication compliance, avoid medication errors, maximize the effect of patients' drug treatment, and ensure the safety and effectiveness of medication. Medication guidance mainly includes:

(1) The name, specification, dosage form and quantity of the drug.

(2) Indications for medication.

(3) Dosage: first dose and maintenance dose. If necessary, the conversion and dosage of the dose should be explained, and for the drugs used "when necessary", the conditions of administration and the limits of medication should be explained.

(4) Medication method: the number or interval of daily administration, and the course of treatment. If there are special requirements for use in the drug instructions, drug guidance shall be given and marked in the drug guidance record.

(5) The expected onset time of the drug, the duration of the drug, and the method of self-monitoring the efficacy of the drug.

(6) The possible results and countermeasures in the event of medication errors.

(7) Common and serious adverse drug reactions, as well as methods to avoid and deal with them.

(8) Precautions between drugs taken at the same time.

(9) Other drugs or dietary contraindications that cannot be used at the same time.

(10) Storage methods, precautions and expiration dates.

(11) Recent revisions to the drug instructions, including the revision and update of the trade name, indications, contraindications, dose, expiration date, storage conditions, and adverse drug reactions.

Article 19 Licensed pharmacists shall carry out drug use guidance as follows:

(1) Introduce yourself to the patient and explain the purpose of the guidance;

(2) Understand the patient's past medical history, current medical history, allergy history and other drug-related information;

(3) Understand the patient's knowledge of diseases and drugs;

(4) Instruct patients to use drugs correctly;

(5) Confirm whether the patient has understood the guidance and recommendations;

(6) Understand the patient's psychological state and improve medication compliance;

(7) Establish objective, standardized, timely and traceable medication guidance records.

(8) When selling prescription drugs and Class A non-prescription drugs, drug guidance should be actively carried out; When selling Class B OTC drugs, medication guidance should be carried out according to the needs of patients.

Article 20 When providing consulting services, licensed pharmacists should listen carefully, ask politely and answer patiently, and should be advised to seek medical treatment in a timely manner for problems whose causes are unknown, may cover up the condition after taking medication, delay treatment or aggravate the condition, or are beyond the scope of their own ability.

Article 21 When the drug is used for children, the elderly, pregnant and lactating women, patients with liver and kidney insufficiency, chronic or special diseases, the licensed pharmacist shall pay close attention to it, make a comprehensive judgment, and give objective guidance to prevent the occurrence of drug accidents.

Article 22 Licensed pharmacists shall provide guidance on drug use in an appropriate manner. Medication guidance mainly includes oral and written materials, physical demonstrations, video and audio, propaganda lectures, telephone or Internet education, etc.

Article 23 Licensed pharmacists shall, in accordance with the national laws and regulations on the reporting and monitoring of adverse drug reactions, collect and report information on adverse drug reactions, and take effective measures to reduce and prevent the occurrence of adverse drug reactions. If necessary, the joint physician responds together to avoid harm to the patient.

Article 24 Licensed pharmacists of drug retail enterprises carrying out drug treatment management shall receive professional training and have the professional quality and service ability to provide patients with information collection, drug education, evaluation and follow-up, and drug adjustment suggestions.

Article 25 The implementation targets of drug treatment management mainly include: those who seek medical treatment or change treatment plans frequently, those who visit multiple departments or have multiple physicians' prescriptions, those who suffer from two or more chronic diseases, those who use five or more drugs, those who are using high-risk drugs or those who have poor compliance.

Article 26 The content of drug treatment management mainly includes information collection, file establishment, evaluation of drug-related problems, joint formulation of intervention plans with physicians and patients, program implementation, follow-up and follow-up, etc.

Article 27 Licensed pharmacists shall publicize disease prevention and health care knowledge to the public, carry out scientific publicity on scientific, rational and safe drug use, and actively advocate a healthy lifestyle. Health education mainly includes:

(1) Carry out popular science lectures related to commonly used medicines, common diseases and healthy lifestyles;

(2) Provide medication consultation and education;

(3) Posting posters, distributing posters and brochures, or playing popular science videos or audio;

(4) Based on the incidence of diseases in the region, carry out targeted publicity and education on the National and World Disease Prevention and Control Publicity Day to prevent and control the occurrence and epidemic of diseases;

(5) Strictly implement the relevant provisions of special management drugs, and promptly stop and inform drug abusers of the harmfulness.

Article 28 Licensed pharmacists shall establish and trace pharmaceutical service records, which shall be complete, objective and standardized. Pharmacy service records (see attached table for reference template) mainly include:

(1) Basic information on patients and information related to drug treatment;

(2) Assess the patient's medication;

(3) The content provided for medication consultation, follow-up follow-up, medication education, and suggestions for patients' healthy lifestyles;

(4) Whether the patient understands and accepts the content of the pharmacy service;

(5) Signature of a licensed pharmacist.

Chapter IV: Code of Conduct

Article 29 When practicing a licensed pharmacist, he or she shall wear a work badge indicating the name, practice qualifications, practice category and other contents of the licensed pharmacist, dress in accordance with the requirements of the health service industry, and have a dignified and tidy appearance.

Article 30 Licensed pharmacists shall abide by discipline and law, be honest and trustworthy, love their jobs and dedicate themselves to their work, practice in accordance with the law, be patient-centered, respect and care for patients, protect patients' privacy, behave in a civilized manner, consciously maintain their professional reputation and social image, respect their peers, help each other in the same industry, compete fairly, improve together, and promote the rational use of drugs by the public.

Article 31 Licensed pharmacists shall be proficient in professional knowledge of pharmacy, participate in continuing education and professional training, carry out regular exchanges on pharmaceutical services, continuously improve service methods, and continuously improve their pharmaceutical service capabilities.

Article 32 When a licensed pharmacist provides pharmaceutical services, he or she shall not engage in any of the following acts:

(1) Recommending or inducing the purchase of drugs that are unrelated to the patient's expression of illness;

(2) Recommending or inducing the purchase of drugs that exceed the treatment needs of the service recipients;

(3) Conducting unscientific publicity, false publicity, exaggerated publicity, deception and misleading;

(4) Introducing and recommending non-drugs in the name of drugs;

(5) Deliberately making inappropriate statements or false promises about possible drug risks;

(6) Inducing service recipients who do not meet the scope of internet hospital diagnosis and treatment services to obtain prescriptions through internet hospitals;

(7) Other prohibited conduct provided for by laws and regulations.
