On April 29, 1945, the USS Hazelwood DD-531, a Fletcher-class destroyer escorting an aircraft carrier in Okinawa waters, was attacked by a kamikaze. The first two Zeros were maneuvered by the Blackswood, and the third, although shot, rushed in from the stern, hitting the second chimney, collapsing the main mast, and finally crashing into the bridge. The bomb carried by the Zero then exploded, burning fuel to cover most of the ship, killing 10 officers and 67 sailors, including the captain, and 36 others missing.

Compare the last standard side view of the Blackswood to see how badly she was hit, but because she didn't hurt the point, she was able to return to Ulrich Atoll on her own power to repair it.
