"For atopic dermatitis, early symptoms are controlled as soon as possible and the rash is cleared. At the same time, maintaining a stable disease or clearing of the skin, thereby improving the quality of life of patients, is the core of standard treatment. On May 8, Gao Gaohua, vice president of the First Affiliated Hospital of China Medical University and director of the Department of Dermatology of the National Key Discipline, said in an interview with reporters that standard treatment is the concept of chronic disease management.

It is reported that this is the first time to put forward a new goal of innovative treatment of atopic dermatitis in China, creating a new era of standard treatment of atopic dermatitis. This year, the exploration of "standard treatment of atopic dermatitis in China" was launched. The aim is to provide proactive, flexible, and sustained disease interventions based on the individualized needs of patients and with the goal of rapid and sustained remission to help patients maximize benefits.

The disease of atopic dermatitis is increasing year by year

In recent decades, the number of patients with atopic dermatitis on the mainland has increased by 25.65%. The prevalence in adults and adolescents is approximately 4.6% and 2.5%, respectively.

Many people know about eczema, but not much about atopic dermatitis. Xingxinghua explained to reporters that in fact, the clinical manifestations of atopic dermatitis are similar to eczema, but atopic dermatitis also has some characteristics of its own:

1. Skin itching, a variety of rashes, infancy, childhood, adolescence and adulthood, the rash performance in old age is not the same.

2. In addition to skin manifestations, some of them are also combined with allergic rhinitis, allergic asthma, allergic esophagitis, etc., which are classified as atopic dermatitis diseases.

Gao Xinghua pointed out that for the diagnosis and treatment of atopic dermatitis, there are some norms and standards both internationally and domestically.

It is worth noting that in the treatment, we must consider the individual situation of the patient, the use of different treatment options, such as simple, mild atopic dermatitis, in addition to the basic skin care, generally use moisturizing supplies, topical preparations may be able to effectively control symptoms; for moderate or severe patients, it is often the use of systematic treatment methods to effectively control the disease.

At present, it is at the turn of spring and summer, pollen, dust mites and other induces the acute onset of atopic dermatitis, and experts also recommend that patients actively go to medical institutions for treatment.

There are challenges in the treatment of atopic dermatitis

"Atopic dermatitis is a chronic disease with a long course of illness, and it is difficult to do so if it is expected to be cured after a drug or a phase of medication." Gao Xinghua said, "In addition, due to the severe skin itching of atopic dermatitis, the quality of life has decreased, and even work, rest and life have been seriously affected." This is the primary concern in treatment, so I think that the first thing to diagnose and treat this disease is to solve the discomfort caused by severe itching during the acute attack, and the second is that the disease requires a period of maintenance treatment. ”

Atopic dermatitis can be said to be onset in any part of the body, if the rash starts on the face or hands, those exposed parts will bring additional psychological burden to the patient, because it will have an impact on the image, such as facial redness, small pimples, blisters, scabs or chronic skin lesions with some skin hypertrophy and roughness, pigmentation, which will also bring additional psychological pressure to the patient.

Therefore, Gaoxinghua believes that in addition to relieving itching, it is also necessary to consider the control of the rash.

Professor Wang Hua, director of the Department of Dermatology at the Children's Hospital Affiliated to Chongqing Medical University, pointed out: "In recent years, although atopic dermatitis has made great progress in clinical treatment, it is often because of the patient's lack of awareness of the disease that leads to repeated and delayed conditions, and the treatment is not standardized enough to increase the burden of disease. For patients with moderate to severe AD to quickly relieve itching, the application of abucitinib tablets, a highly selective JAK1 inhibitor, is expected to help more patients with moderate and severe AD get rid of disease troubles and return to normal work and life as soon as possible. ”

Standard treatment has two core connotations

The concept of "standard treatment" has long been widely used in many areas of chronic diseases, such as diabetes, hypertension, and hyperlipidemia. In the field of inflammatory diseases, such as rheumatoid arthritis, psoriasis, etc., and the standard treatment concept of atopic dermatitis has just been proposed internationally, is this concept applied to the management of AD patients in China?

Xingxinghua believes that standard treatment is a concept of disease management, especially for chronic diseases, for atopic dermatitis, early symptoms should be controlled as soon as possible and rashes should be cleared. At the same time, maintaining the state of disease clearance or skin clearance, thereby improving the quality of life of patients, which is the core of standard treatment.

For patients with atopic dermatitis, it is necessary to achieve standard treatment, especially in patients with a more serious degree of disease, first of all, the control of itching, followed by the control of rash, these two aspects are complementary. Rapidly control symptoms, and then maintain treatment according to the situation, so that the patient's condition is in a state of basic stability or almost no lesions and no discomfort symptoms.

Professor Ma Lin, Director of the Department of Dermatology at Beijing Children's Hospital affiliated to Capital Medical University, said: "Atopic dermatitis is the most common chronic, recurrent, inflammatory, and pruritus skin disease, and is the first major disease of children's skin. If the treatment is not good, it will affect the growth and development of the child and even the quality of life of the whole family. AD is the first window in the allergic process, and managing atopic dermatitis is essential for reducing asthma, allergic rhinitis, conjunctivitis or the course of the disease in the future. Reasonable and standardized application of ladder, systematic, and sequential treatment can help alleviate the recurrence of atopic dermatitis and improve the quality of life of patients. ”

According to the reporter's understanding, at present, many dermatology hospitals across the country have set up specialized outpatient clinics for atopic dermatitis.

Xingxinghua said that the number of dermatology outpatients is often one of the clinics with the largest number of outpatients in hospitals, there are many patients, different types of diseases, how to better serve patients? For example, the atopic dermatitis clinic, led by a specialist to set up a special disease clinic, set up a day or two days a week for atopic dermatitis patients for reasonable diagnosis and treatment, disease management and maintenance.

In this way, the experience of patient management is also richer, and the efficiency of management will be higher. In addition, through the training of young doctors or the grass-roots level, the concept and technology of disease management are popularized, so as to form a system for the treatment of diseases.
