What is the best age for a man and a woman to have children? The answer is finally clear!

Theoretically, there is, and medicine generally believes that women's 25 to 34 years old is the best age for childbearing. And this "best" can be understood mainly from two aspects.

On the one hand, there is the fertility of women, that is, the probability of being able to "conceive".

It is currently believed that women's fertility will decline against the increase in age, especially after the age of 35, and overall, the older the age, the lower the fertility. Although there are some exceptions, very few women after the age of 43 are able to conceive naturally and have a successful live birth [[1].

Fertility refers to the probability of conceiving in a menstrual cycle, and the decline in fertility may be related to a variety of factors.

When a woman is born, the number of oocytes is certain and decreases with age. The quality of oocytes also decreases with age, as the more frequently meiosis abnormalities occur as they age[[2], the number of chromosomally abnormal oocytes in ovulation increases, leading to decreased fertility and an increased risk of spontaneous miscarriage.

There have been many studies that have found that the older the woman, not only the probability of natural conception is reduced, but also the probability of successful pregnancy during the artificial cycle or artificial insemination is also lower.

For example, a classic study of women undergoing artificial insemination due to male infertility (azoospermia) found that women aged ≥ 35 had lower fertility rates and required more cycles to achieve conception [3]].

Among them, the fertility rate of women aged 26-30 and women aged 25 and under is similar, while the success rate of conception for women aged 31-35 is moderate. The fertility rates for women under the age of 31, 31-35 and over are 74%, 62% and 54%, respectively.

On the other hand, there are problems and various complications that women may encounter during the fertility process, as well as abnormalities and poor prognosis of the fetus.

Older women have a higher rate of spontaneous miscarriages during the first trimester, with the vast majority of miscarriages occurring at 6 to 14 weeks' gestation [20]. Among the most common causes of these miscarriages are chromosomal abnormalities, both triploid and holploid, mainly caused by a decrease in oocyte quality, and changes in uterine and hormonal function may also play a certain role.

In addition, women aged ≥35 years of age have a 4- to 8-fold risk of ectopic pregnancy compared to younger women [a4]. At the same time, the risk of chromosomal abnormalities in the fetus, especially aneuploidy, shows a steady increase with the age of women [5].

Older women also have an increased risk of pregnancy-related complications, such as a population study that included nearly 830,000 singleton births in Washington State, which showed that women aged ≥ 40 years of age had an eight-fold increased risk of amniotic fluid embolism and a three-fold risk of obstetric shock compared with women aged 25 to 29 years.

The two most common medical complications of pregnancy in women, hypertension and diabetes, increase in older women, especially obese women, as do placental problems. The incidence of preeclampsia is 3% to 4% in the general obstetric population, increases to 5% to 10% in women over 40 years of age, and as high as 35% in women over 50 years of age [6]. There is also an increased risk of preterm and stillbirth

But then again, is it because of the factors attached to these "optimal reproductive years" that we have to clamor to set a goal, to complete the "reproductive task" before the age of 35?
