"Duck webs" grow in the neck?
The deputy chief physician of the Vascular Surgery Shangtao received Uncle Zhou, combined with his symptoms and previous examination reports, after perfecting a series of examinations such as carotid B ultrasound and carotid digital subtraction angiography (DSA) for him, it was found that Uncle Zhou's carotid artery actually grew a "duck web", and this is the "culprit" that caused Uncle Zhou to have many cerebral infarctions!
Ordinary people's carotid lumen is normal flow of blood, and Uncle Zhou's carotid artery lumen wall grew a film-like sheet, about 1cm * 0 .8cm, under the wash of blood, floating and swinging, like duck webbing, so it is called carotid webbing.
Why does this "duck webbing" cause Uncle Zhou to repeatedly have cerebral infarction?
Experts tell us that this is like a block in the flow of water, when the water flows around it, due to the web swinging back and forth, causing the flow direction and flow rate of the water to change, forming a vortex here, a part of the blood will accumulate here, forming a blood clot for a long time. Because the blood flow in the carotid arteries is supplied to the brain, when the thrombus is detached, it flows along the blood flow to the brain, causing cerebral infarction.
At present, the treatment of carotid webbing is generally to perform open surgery to remove webs, but because Uncle Zhou also suffers from serious heart disease, kidney failure and other diseases, the risk of surgery is very large. "Carotid webbing is actually a developmental abnormality of the arterial lining, the cause is unknown, most people are congenital developmental abnormalities, related to genes and genetics." Professor Zhang Hongkun, director of the Department of Vascular Surgery of the First Hospital of Zhejiang University, said.
Through talking with Zhou Liang, it was learned that his grandfather also died of cerebral infarction, but at that time, no detailed examination was carried out, and it was impossible to determine whether it was a cerebral infarction caused by carotid webbing. Although Zhou Liang himself is only 30 years old, he has often had headaches in recent years, and has done a series of head examinations, but has not found the cause, and he said that he plans to do a carotid artery examination in the near future.
The incidence of carotid webbing is low, perhaps only one or two out of 1,000 people, which is easy to ignore, and the number of confirmed cases has been increasing in recent years with the continuous advancement of imaging technology. It is easy to be misdiagnosed as carotid artery plaque, arteriosclerotic disease, aortic dissection caused by hypertension, etc., experts say that it is necessary to conduct a detailed head and neck blood vessel ultrasound examination as soon as possible to evaluate, if the propensity is carotid webbing, and then combined with angiography or CTA for confirmation.
Just during the Chinese New Year, the Department of Vascular Surgery of the Zhijiang Branch of the First Hospital of Zhejiang University received another case of carotid webbing patient, a 60-year-old male patient who went to many hospitals due to long-term dizziness and fatigue, and was finally diagnosed with carotid webbing in the First Hospital of Zhejiang University.
Experts say that in general, carotid webbing does not require special treatment if no obvious symptoms are shown, but follow-up should also be noted. If a condition such as blurred vision, headache or even stroke is triggered, it needs to be treated with drugs or even surgery. Patients with uninjusted cerebral infarction, especially those with recurrent cerebral infarction, should be wary of the possibility of carotid webbing.
