"I really didn't expect to almost die of fat, this time I decided to lose weight!" Mr. Liu, who weighs 230 pounds, suddenly suffered respiratory failure when he slept at night, and his loss of consciousness was hanging on the line. Fortunately, he was sent to the hospital in time, and underwent positive airway pressure ventilation treatment at the Sleep Disease Diagnosis and Treatment Center of Wuhan Sixth Hospital (Affiliated Hospital of Jianghan University), and after escaping death, he assured the doctor that the top priority was to lose weight.

Mr. Liu, 31 years old this year, is the only son in the family, his parents have been pampered since childhood, eating and drinking are saved for him, and his weight is obviously higher than that of his peers. In recent years, Mr. Liu has often worked outside the home, and his height of 1.65 meters has soared to 230 pounds. With the increase in weight, Mr. Liu's spirit is getting worse and worse, he is groggy during the day, he is inattentive, he always wants to sleep, and he sleeps at night with a thunderous snoring sound, making it difficult for his family to sleep.

On the evening of December 24, the family found that Mr. Liu, who was purring and shaking, suddenly had no sound for a long time when he slept, and shouted that he had no response, and the family quickly called 120 to send him to the emergency department of the Sixth City Hospital. The examination found that Mr. Liu had severe respiratory failure, blood oxygen saturation was as low as 50%, the partial pressure of carbon dioxide was as high as 117.6mmHg, and his condition was critical, and he was immediately transferred to the intensive care unit (ICU) of the hospital, and woke up after emergency rescue.

After consultation with Dr. Jiang Cheng, director of the Sleep Disease Diagnosis and Treatment Center, considering that Mr. Liu had obesity hypopnea syndrome and respiratory failure, he transferred the patient to the Respiratory Intensive Care Unit (RICU) for polysomnography monitoring. "Mr. Liu constantly has apnea all night, multiple organs are in a state of hypoxia, and the minimum value of blood oxygen is only 40%." Director Jiang Cheng diagnosed that Mr. Liu not only suffered from obesity hypopnea syndrome, but also had severe obstructive sleep apnea hypopnea syndrome (OSAHS).

"Mr. Liu due to excessive obesity led to airway narrowing, causing sleep apnea syndrome, he often snored when he slept, prone to apnea, hypoxemia; in addition, Mr. Liu's obesity limits lung expansion, insufficient ventilation during breathing, carbon dioxide in the body can not be discharged, aggravation of the disease leads to coma. This is the most common cause of respiratory failure in obese people. Director Jiang Cheng decided to give Mr. Liu a double-level ventilator for positive airway pressure ventilation, and adjusted the inspiratory pressure and expiratory pressure of the ventilator to the most suitable level for the patient through the manual pressure regulation mode. The air pressure generated by the ventilator acts as a support for the airway, while the pressure difference between the two horizontal pressures increases the patient's ventilation volume and can quickly reverse respiratory failure. After three days of treatment, Mr. Liu's spirit improved significantly, and his blood oxygen saturation and carbon dioxide level in his body returned to normal.

Director Jiang Cheng introduced that patients with obese hypopnea syndrome usually have a 90% chance of suffering from obstructive sleep apnea hypopnea syndrome at the same time, and patients with this disease are prone to sudden death at night. He reminded that when there is snoring, holding breath at night, poor sleep quality, easy sleepiness during the day, etc., you should go to the hospital in time for detailed examination and timely treatment to avoid tragedy.
