It wasn't until December 3 that people learned about the real scene of "soul bargaining" through a video. After 8 rounds of competition, which lasted for one and a half hours, the market price of 697,000 yuan per shot of rare disease drug Nocinasin sodium injection was finally cut to less than 33,000 yuan per injection by the negotiation expert group led by Zhang Jinni. Whether a drug can be reimbursed is determined by the national medical insurance drug list. After the establishment of the National Medical Insurance Bureau in 2018, the adjustment period of the national medical insurance drug list was shortened from a maximum of 7 years to 1 year. At the same time, it is stipulated that for exclusive drugs with high clinical value, breakthrough efficacy but expensive prices, they must be negotiated to be included in the medical insurance directory, which is the so-called "every advance must be discussed".

In the first round of drug negotiations of the National Medical Insurance Bureau in 2018, 17 drugs were successfully negotiated, involving more than 10 kinds of cancer treatment drugs, with an average reduction of 56.7%; in 2019, 70 drugs were successfully negotiated, including 22 high-priced anti-cancer drugs including PD-1, with an average reduction of 65%; the third round of drug negotiations in 2020 included 96 exclusive drugs, and the average price of drugs fell by more than half; this year's new version of the national medical insurance drug list added 7 rare disease drugs in one go. Patients urgently need drugs, pharmaceutical companies urgently need to open the market, the National Medical Insurance Bureau acts as a "super buyer" on behalf of the insured, and drug negotiations begin.
However, the "soul bargaining" is only one stage of drug negotiation, and the entire drug negotiation process often lasts for half a year, which is divided into five stages: preparation, declaration, expert review, negotiation, and announcement of results. "The workload is extremely huge, and there are many departments that need to communicate, but everyone is holding their hearts, that is, they want the people to use medicines and afford medicines as soon as possible." An expert who has been involved in the negotiations for a long time said.
The adjustment of the national medical insurance drug list in 2021 will be launched from May, and the preparatory stage will mainly study and draft the adjustment plan, improve relevant norms and mechanisms, and establish an expert team. At the declaration stage, the main organization guides enterprises to make declarations and submit drug-related information in accordance with procedures, organizes a team of experts to review the declaration information and publicizes it, and feedbacks the review results to the corresponding enterprises to form a list of drugs that have been successfully declared.
After the list of drugs was formed, the "battle" began. In the expert review stage, a key task is to measure the negotiated drug by experts and determine the negotiated reserve price.
How is the negotiated reserve price formed? Xu Wei, director of the Pharmaceutical Service Management Division of the Zhejiang Provincial Medical Security Bureau, who has long participated in drug negotiations, revealed that the calculation team often has two groups of people, one group of experts is basically based on pharmacoeconomics experts, according to the efficacy and the price of similar products at home and abroad to measure the real price of drugs; the other group of experts is the medical insurance department personnel, according to the medical insurance fund for comprehensive stress testing. The two working groups calculated in parallel, independently gave reference prices in complete secrecy, and finally determined the negotiated reserve price of each drug.
"During the review period, all experts gathered in the Great Hall of the People Hotel closed, all participants signed confidentiality agreements and letters of responsibility, and it was strictly forbidden to exchange the content of the review with each other. The negotiation team is generally composed of 1 negotiation team leader and 4 staff members, and on the day of the negotiation, the negotiators draw lots on the spot to determine the negotiation venue for the day. Before the negotiation begins, the negotiator will receive an envelope containing the name of the drug under negotiation and the negotiated reserve price, and after reading out the negotiation rules, the negotiation officially begins. According to the current negotiation rules, the first quotation is made by the enterprise side, and the enterprise side has two opportunities to quote and confirm. If the price after the second confirmation of the enterprise is still higher than 115% (exclusive) of the negotiated reserve price of the medical insurance party, the negotiation fails and is automatically terminated.
