Grade 4: may be malignant, with one or more features of a malignant nodule, which can be subdivided into 4a, 4b, and 4c, and the probability of malignancy is gradually increased; close follow-up (usually every 3 months) or further evaluation is required, including ultrasound-guided fine needle biopsy, which may require surgery.
Grade 5: High suspicion of malignancy (80% to 90% probability) requires further evaluation (e.g., fine needle puncture) or surgery.
Grade 6: Thyroid cancer that has been confirmed by fine needle aspiration.
